RATLARDA FARKLI DOZLARDA İBANDRONİK ASİT KULLANIMININ DİŞ ÇEKİMİ SONRASI ALVEOLER SOKETE ETKİSİNİN HİSTOPATOLOjİK OLARAK DEĞERLENDİRİLMESİ

Özet
Amaç: Bu çalışmanın amacı farklı dozlarda ibandronik asit kullanımın diş çekimi sonrası yara iyileşmesine etkisinin değerlendirilmesidir. Gereç ve Yöntem: Bu çalışmaya toplam 30 adet dişi Sprague- Dawley rat dahil edildi. Deney hayvanları 3 gruba ayrıldı: Grup 1’e dahil edilen hayvanlara 0.033 mg/kg ibandronik asit, Grup 2’ye dahil edilen hayvanlara 0.1 mg/kg ibandronik asit ve Grup 3’e dahil edilen hayvanlara distile su 6 haftalık deney sürecinde subkütan yol ile uygulandı. Son enjeksiyon işlemi tamamlandıktan 5 gün sonra bütün hayvanlarda mandibular molar diş çekimi yapıldı. Postoperatif 8. haftada deney hayvanlarının sakrifikasyonu yapıldı. Ratların sakrifikasyonunu takiben hemimandibulektomi ile doku örnekleri elde edildi ve bu örnekler histopatolojik inceleme için hazır hale getirildi. Histopatolojik bulgular istatistiksel olarak analiz edildi. Bulgular: Bütün gruplarda yeni kemik oluşumu görüldü. Osteoklastik ve osteoblastik aktivite bütün gruplarda artış gösterdi. Grup 1’de ve Grup 2’de enflamasyon oranı Grup 3’te görülen enflamasyon oranından istatistiksel olarak anlamlı derecede yüksek bulundu. Grup 2’de görülen mukozal ülserasyon Grup 1 ve kontrol grubundan istatistiksel olarak anlamlı derecede yüksek bulundu. Grup 2’de yer alan ratlardan 3’ünde (%37.5) osteonekroz bulguları görüldü. Osteonekroz oranları bakımından gruplar arasında istatistiksel olarak fark olduğu görüldü (p< 0.05). Sonuç: İbandronik asit ratlarda diş çekimi sonrası oluşan yara bölgesinde inflamasyon oranında önemli derecede artışa neden olmuştur. İbandronik asitin kümülatif dozunda artış ise (0.1 mg/kg/hafta’ya karşı 0.033 mg/kg/hafta) yumuşak doku ülserasyonu ve osteonekroz gelişme oranında anlamlı bir artış ile sonuçlanmıştır.
Abstract
Background and Aims: The aim of this study was to investigate the effects of different doses of ibandronic acid on alveolar wound healing after tooth extraction. Material and Methods: Thirty female Sprague-Dawley rats were used in this study. The rats were divided into three groups: Group 1 received 0.033 mg/kg of ibandronic acid, Group 2 received 0.1 mg/kg of ibandronic acid and Group 3 received distillate water; all of the treatments were administered via subcutaneous injections for six weeks. The mandibular molars were extracted from each group five days after the last dose of the drugs and the placebo. The rats were sacrificed eight weeks postoperatively. Hemimandibles were removed and prepared for histopathological examination following scarification of the rats. The histopathological findings were statistically analysed. Results: New bone formation was observed in all groups. Osteoblastic and osteoclastic activity also increased in all three groups. The rate of inflammation was significantly higher in Group 1 and Group 2 than in Group 3. Mucosal ulceration in Group 2 was significantly higher than in Group 1 and the control group. Osteonecrosis was seen in three of the rats from Group 2 (37.5%); this result was statistically significant. (p< 0.05) Conclusion: Ibandronic acid caused a significant inflammatory change in rat mandibles after tooth extraction. Increase in cumulative dose (0.1 mg/kg/week vs 0.033 mg/kg/week) of ibandronic acid triggered significant soft tissue ulceration and osteonecrosis.
