Deneysel Akut Pankreatit Modelinde Adalimumab’ın Doku Hasarını Önleyici Etkisi

Özet
Amaç: Tümör nekroze edici faktör alfa (TNF-α), akut pankreatit patogenezinde ve komplikasyonların gelişiminde anahtar role sahiptir. Çalışmanın amacı, ratlarda oluşturulmuş şiddetli akut nekrotizan pankreatit modelinde TNF-α inhibitörü olan Adalimumab’ın (ADA) etkinliğinin araştırılmasıdır. Yöntem: 70 adet Wistar albino cinsi dişi rat Grup 1: SHAM laparotomi, Grup 2: Modifiye SHOP modeli ile şiddetli nekrotizan pankreatit (MS-SNP), Grup 3: MS-SNP ve intraperitoneal (İP) salin, Grup 4: ADA 2 mg/kg İP + MS-SNP, Grup 5: ADA 4 mg/kg İP + MS-SNP, Grup 6: ADA 4 mg/kg İP ve Grup 7: ADA 2 mg/kg İP olarak gruplandırıldı. MS-SNP; ortak safra ve pankreas kanalınının duodenuma girmeden önce bağlanması ve 50 μg/kg cerulein İP verilmesiyle oluşturuldu. Ratlar AP oluşturulmasını takiben 24. saatte sakrifiye edilerek; serum pankreatik amilaz ve pankreas dokusunda ödem, nekroz, inflamasyon, yağ nekrozu ve kanama parametreleri değerlendirildi. Bulgular: Deneysel ADA etkinliğini araştırdığımız Grup 4 ve 5 ile Grup 2 kıyaslandığında pankreatik ödem, inflamasyon, nekroz, kanama, yağ nekrozu ve total patoloji skorları düşük tespit edilmiş, Grup 5’te tüm parametreler açısından anlamlı farklılık elde edilmiştir (P< ,05). Grup 4’te ise yalnızca pankreatik ödem, yağ nekrozu ve total patoloji skorlarındaki düşüklük anlamlı bulunmuştur. Pankreatik amilaz değerlerinde anlamlı düşüş saptanmamıştır. ADA’nın uygulanan dozlarda tek başına pankreatit yapmadığı da gösterilmiştir. Sonuç: ADA uygulanan grupta pankreatik ödem, nekroz ve kanama gibi histopatolojik değişikliklerin oluşması engellenmiş ve histopa- tolojik skorlar ADA uygulanmayan gruplara göre anlamlı derecede düşük saptanmıştır. Gelecekte ADA’nın AP tedavisinde kullanımıyla ilgili ümit verici sonuçlar elde edilmekle birlikte bu amaca yönelik farklı modeller ve farklı ADA dozlarıyla benzer çalışmalara ihtiyaç vardır
Abstract
Objective: Tumor necrosis factor alpha (TNF-α) has a central role in acute pancreatitis pathogenesis and its related systemic compli- cations. The aim of this study is to evaluate the therapeutic effectiveness of TNF-α inhibitor adalimumab (ADA) in severe necrotizing pancreatitis (SNP) model in rats. Methods: Seventy female Wistar albino rats were randomly divided into the following groups; group 1: SHAM laparotomy, group 2: SNP with modified SHOP model (MS-SNP), group 3: MS-SNP and saline intraperitoneally (IP), group 4: ADA 2 mg/kg IP + MS-SNP, group 5: ADA 4 mg/kg IP + MS-SNP, group 6: ADA 4 mg/kg IP and group 7: ADA 2 mg/kg IP. MS-SNP was induced by complete pancreatic duct obstruction and cerulein hyperstimulation (50 μm/kg IP). The subjects were sacrificed at the 24 th hour, and then pancreatic amylase and edema, necrosis, inflammation, fat necrosis and bleeding parameters were evaluated. Results: When Group 4 and 5 were compared to Group 2, all histopathological scores were decreased. In group 4, only pancreatic edema, fat necrosis and total pathology scores were significantly lower than those of Group 2, on the other hand, all these parameters were significantly lower for Group 5 (P < .05). Pancreatic amylase levels were comparable between groups Conclusion: Histopathological changes such as pancreatic edema, necrosis and bleeding were prevented by the administration of ADA, and the total pathology scores were found to be significantly lower in rats receiving ADA than the ones without ADA. The potential therapeutic role of ADA in acute pancreatitis should be evaluated in future studies.
