Özet
Ege Üniversitesi Tıp Fakültesi Genel Cerrahi ABD'da 1995-1998 yılları arasında mide stromal tümörlü beş olgu öpere edildi. Kadın olgu sayısı üç, erkek sayısı iki iken, ortalama yaş 50 idi. Olguların dördüne tam endoskopi sırasında makroskopik görünümleri ile, birine operasyon esnasında kondu. Endoskopik biopsiler, hastaların hiçbirinde, histopatolojik olarak tanıyı desteklemedi. Bir olguya wedge rezeksiyon uygulanırken, diğer olgulara parsiyel mide rezeksiyonu uygulandı. Bir hasta postop erken dönemde pulmoner emboli nedeniyle kaybedilirken, diğer olguların ortalama izlem süresi 6 aydı. Gastrointestinal mesenkimal tümörler günümüze kadar özellikle leiyomyomatöz lezyonlar olarak değerlendirilmiştir, ancak immunhistokimyasal çalışmalarla bu tümörlerin hücre düzeyinde diferansiasyonları gösterilmiş ve stromal tümörlerle diğer tümörler arasında kesin ayırım sağlanabilmiştir. Bu çalışmada özellikle immunhistokimyasal çalışmalar ve stromal tümörlerde gelinen son nokta konusunda tartışmayı uygun gördük.
Abstract
Between 1995-1998, we had five cases of gastrointestinal stromal tumor(GIST) of the stomach in General Surgery Department of Ege University. There were three female, two male patients with median age 60. Diagnosis were performed with macroscopic appearance of tumor by endoscopy in four patients but none of them had histopathological confirmation with endoscopie biopsy. Wedge resection was performed for one patient and partial gastric resection were performed for others and the tumors had the diagnosis as GIST by histological and immnohistochemical examination of the operative specimens. There was only one mortality in the early postoperative period due to pulmonary embolism. Median follow-up period was six mounth. Mesenchymal tumors of the gastrointestinal tract have been traditionally regarded as largely leiomyomatous lesions. However GIST and the other tumors have been distinguished recently on the basis of tumor cell differ antiation shown by immunohistochemical studies. In this report we discuss the concept and the immunohistochemical characteristics of GIST.
