Özet
Bilateral meme kanseri tanısı için; karşı taraf meme kanserinin histolojisinin farklı olması, uzak metastazlarla birlikte olmaması, karşı memede insitu değişikliklerin bulunması, histolojik diferansiasyonun daha yüksek olması gibi kriterlerin bulunması gerekir. 1987-1998 yıllan arasındaki 11 yıllık dönemde kliniğimizde ameliyat edilen ve izlenen 120 olgu bu kriterlere göre değerlendirildi. 9 olguda (%7.5) bilateral meme kanseri saptandı. Olguların 4'ü senkron, 5'i metakrondu. 6 olguda (%66.6) erken evre tümörlere (TNM I-II), üç olguda (%33.3) geç evre tümörlere rastlandı. İki primer kanser arasındaki süre 2-9 yıl olup ortalama 3.3 yıl idi. Hiç bir olguda uzak metastaz yoktu. İki olguda farklı histolojik yapı mevcuttu. Modifiye radikal mastektomi (MRM) + Radyoterapi (RT) + Kemoterapi (KT) uygulanan olgular çeşitli parametrelerle (nodal status, çap, insutu değişiklikler, invazyon, evre) değerlendirildiğinde; literatürde histolojik grade, büyüklük, ilk tümör büyüklüğünün risk faktörü olup olmadığı kesin bilinmemesine rağmen, erken evreye uyan olgularda daha yüksek oranlarda bilateral meme kanseri riski saptandı. Bu nedenle yakın izlem ve erken tanı bu olguların prognozunda büyük önem taşımaktadır.
Abstract
In the diagnosis of bilateral breast cancers, the criterias; different type of histology in the other breast, no distant metastasis, insitu changes and increased histologie differantiation in the contralateral breast must be considered. The 120 cases which were followed between 1987-1998 in our clinic with the diagnosis of breast cancer were evaluated according to the above criterias. Bilateral breast cancer was found in nine cases (% 7.5). Four case was synchronous other five cases were metachronous type cancers. In six cases (%66.6 ) the tumors were in early phase (TNM I-II), in two cases the tumors were in the late phase. The period between two primary cancers was 2-9 years(mean 3.3). There were no distant metastasis in all cases. In two cases the histologie structures were different. In these cases modified radical mastectomy (MRM) + radiotherapy (RT) + chemoterapy (CT) were applied and when evaluated by various parameters (i.e nodal status, size, insitu changes, invasion, grade), although in the literature it is not clearly known wheter the histologie grade, size and first tumor size are risk factors, in cases who were in early phase increased risk of bilateral breast cancer was observed. For this reason, close follow-up and early diagnosis of cases is very important.
