Diagnosis and treatment of ruptured hydtid cysts of the lung ( A 20 year experience )

Özet

Akciğer hidatik kistinin plevral alana veya bronşiyal sisteme rüptürü nadirdir. Ancak kist rupture olduğunda hayatı tehdit eden ve çoğu zaman da acil müdahele gerektiren komplikasyonlar gelişebilir. Kliniğimizde son 20 yıllık dönemde kist rüptürlü 54 hasta tedavi edildi. Rüptürlerin 23'ü (%42.6) plevral alana 31'i (%57.4) bronşiyal sisteme idi. Tanıda çoğunlukla göğüs röntgenografıleri yeterli iken (%90) bazı hastalarda komputerize tomografi, ultrasonografı ve bronkoskopi gibi ek inceleme yöntemleri gerekti. Tüm olgular cerrahi yolla tedavi edildi. 46 hastaya kistektomi + kapitonaj ve 2 hastaya ciddi pulmoner hasar ve enfeksiyon nedeniyle lobektomi gibi radikal cerrahi girişimlerde bulunuldu. Mortalite yoktu.

Abstract

The rupture of the pulmonary hydatid cyst into the pleural space or into the bronchial tree is rare. However, life threatening complications requring emergency surgery may develop. During in the last 20 years, 54 patients with hydatid cyst rupture. Of these 23 (42.6 %) were in to the pleural space and 31(57.4%) into the bronchial tree. Chest X-ray was suffucient (90%) for diagnsis in most cases but additional diagnostic methods such as computerized tomography, ultrasonography and bronchoscopy were necessary in some patients. All of the cases were treated surgically while 2 patients with serious pulmonary damage and infections had to be treated with a more radical surgical approach, i.e. lobectomy. No mortality occured in our series.
