Özet
Kolorektal karsinomlar için uygulanan cerrahi girişimler sırasında kolon ve rektumda birden fazla karsinom odağına rastlanması nadir bir durum değildir. Kolon karsinomlannda senkron odakların insidansının belirlenmesi amacıyla İzmir Atatürk Eğitim Hastanesi 2. Cerrahi Kliniğinde 1990-1998 yılları arasında kolon kanseri tanısı ile cerrahi girişim uygulanan 97 olgu retrospektif olarak değerlendirildi. İncelenen 97 olgunun dördünde (%4.2) senkron karsinom odakları gözlendi. Üç olguya tanı preaperatif dönemde kolonoskopi ve çift kontrast kolon grafisi ile konulurken birine de acil şartlarda operasyon esnasında konuldu. Üç hastada senkron odaklar kolonun komşu olmayan segmentlerinde yerleşmekteydi ve bu da cerrahi işlemin boyutlarının genişlemesine neden oldu.Kolorektal karsinomlu olgularda preoperatif dönemde tüm kolonun öncelikli olarak kolonoskopi ile değerlendirilmesinin ve cerrahi planın buradan elde edilen bilgiler ışığında yapılmasının cerrahi tedavinin etkinliğini arttıracağını düşünüyoruz.
Abstract
The finding of more than one site of carcinoma of the colon and rectum at the time of operation is nor unusual. Ninety-seven cases were operated on with a diagnosis of colorectal carcinoma at İzmir Atatürk State Hospital 2. Surgery Department between 1990-1998 were analysed retrospectively to find the synchronous colon cancer. Synchronous colon cancer was seen in four of the 97 cases (%4.2). As three cases were diagnosed in the preoperative period with colonoscopy or double-contrast barium enema, the other one diagnosed in emergency operation for bowel obstruction. In three cases the lesions were localised in different segment of the colon and this different localisation yields important changes in the operative strategy. We believe that the efficiency of the surgical treatment of colorectal carcinoma will increase by using the colonoscopy in the preoperative period and considering this information in determining the operative strategy.
