Özet

Açık kolesistektomi sonrası ağrının önlenmesinde, uzun etkili lokal anestezik madde olarak bupivakainin mi, yoksa periferik ve merkezi etkili nonsteroid antiinflamatuar ilaçlardan olan piroksikam uygulamasının mı daha etkili olduğunu araştıran prospektif randomize çalışmadır. Birinci gruptaki 15 hastaya klasik açık kolesistektomiden sonra tabakalar kapatılırken peritona, fasciaya, cilt altına ve cilde toplam 40 ml % 0.25 lik bupivakain enjekte edildi. İkinci gruptaki hastalara ise, açık kolesistektomi ameliyatı tamamlandıktan sonra, 40 mg piroksikam intramuskuler uygulandı. Enjeksiyondan 24 saat sonra 20 mg piroksikam tablet verildi. İki gruptaki hastaların ağrıları ameliyat sonrası L, 2., 6., 12., 24. ve 48.nci saatlerde verbal rating skala ile değerlendirildi. Sadece ameliyat sonrası 1. saatte ağrı, bupivakain grubunda anlamlı olarak daha azdı (p=0.037). Diğer saatlerde iki grup arasında anlamlı bir farklılık yoktu. Gerek lokal anestezik bupivakainin, gerekse sistemik uygulanan nonsteroid antiinflamatuar piroksikamın açık kolesistektomi sonrası ağrıyı önlemede yeterli olmadığı anlaşıldı. Sadece ameliyat sonrası 1. saatte bupivakain daha etkili olmakla birlikte, birbirlerine anlamlı bir üstünlükleri yoktur.

Abstract

This is a prospective randomized study which compares the effects of long acting local anaesthetic agent bupivacaine and a nonsteroid antiinflammatory drug piroxicam in relieving postoperative pain after open cholecystectomy. In the first group (n=15) 40 ml of 0.25% lignocaine were injected into the peritoneum, fascia, subcutis and dermis at the end of classical open cholecystectomy. In the second group (n=15) intramuscular piroxicam 40 mg was used immediately at the completion of surgery. Piroxicam tablet 20 mg was given to the patients once 24 hours after injection. Postoperative pain was evaluated by using verbal rating scale at postoperative 1st, 2nd, 12th, 24th, and 48th hours in two groups. Pain was significantly less (p=0.037) in patients who received bupivacaine only at postoperative 1st hour. There was no difference between two groups at the other hours. Neither local anaesthetic bupivacaine nor systemic nonsteroidal antiinflammatory piroxicam were successful to relieve the pain after open cholecystectomy. Bupivacaine was found more effective than piroxicam only at postoperative 1st hour, but one of these methods is not superior to the other one.
