Mide adenokarsinomlarında cerrahi yaklaşımımız ve sağkalım

Özet
SSK Tepecik Hastanesi 2. Cerrahi Kliniğinde 1991-97 yılları arasında 104 mide adenokarsinom olgusu öpere edildi. Gastrektomi ve rekonstrüksiyon yöntemleri, tümör lokâlizasyonu, evreleme ve sağkahm açısından değerlendirildi. -Olguların 74 (%71.1)'ü erkek, 30 (%28.8)'u kadın, yaş ortalamaları 55 idi. . Tümör lokâlizasyonu, olguların %58.7 'sinde antrum, % 22.1'inde korpus, %7.7'sinde kardia, % 10.6'sında diffüz, %0.9'unda özofagogastrik bileşkede idi. Olguların 8 (%7.7)'ine total gastrektomi+ splenektomi +jejunal poş, 1 (%0.9)'ine total gastrektomi, 10 (% 9.6)'una total gastrektomi+splenektomi+Roux-Y, 4 (% 3.9) 'üne proksimal subtotal gastrektomi-^ splenektomi, 8 (% 7.7) 'ine distal subtotal gastrektomi (Billroth I), 51 (% 49) 'ine distal subtotal gastrektomi (Billroth II) ve 20 (% 19.2) olguya çeşitli palyatif işlemler yapıldı. TNM evreleme sistemine göre, olguların % 7.6'sı I, % 15.4'ü II, % 46.1 III. ve % 30.8 IV. ievredeydi. İki yıllık sağkahm oranlarımız, Evre I'de % 80, Evre II'de % 70, Evre III'de % 32.1 ve Evre IVde % O'dır. Sağkahm oranlarımızın düşüklüğü göz önüne alınırsa, semptomlar ortaya çıkmadan yapılacak tarama çalışmaları ve nonspesifık şikayetlerin ayrıntılı olarak incelenmesi, uygun cerrahi rezeksiyon ve lenf disseksiyonu ile daha uzun sağkahm mümkündür.
Abstract
In the years 1991-97, 104 cases of Gastric Adenocarcinomas were operated on at the T Surgical Clinic,- SSK Tepecik Hospital, Izmir, in Turkey. They are evaluated in this report with reference to methods of gastrectomy and reconstruction used, location of the tumour, stage of the tumour and survival of the patient. The cases comprised 74 (71.1%) males and 30 (28.8%) females with an average age of 55. A total gastrectomy and splenectomy was performed in 8 (7.7%) cases; total gastrectomy, splenectomy and jejunal pouch were required in 2 (1.9 %) cases, total gastrectomy in 1 (0.9 %) case; total gastrectomy, splenectomy and Roux-Y in 10 (9.6 %) cases; proximal subtotal gastrectomy and splenectomy in 4 (3.9%) cases; distal subtotal gastrectomy (Billroth I) in 8 (7.7%) cases; distal subtotal gastrectomy (Billroth II) in 51 (49 %) cases and various palliative methods were employed in 20 (19.2 %) cases. Location of tumours were.as follows': 58.7 % in the antrum; 22.1 % in the corpus; 7.7 % in the cardia; 10.6% were diffused and 0.9 % in the oesophagogastric junction. According to the TNM staging system, 7.6 % of the cases were at Stage I, 15.4 at stage II, 46.1 %at stage III and 30.8% at stage IV. Our survival rates for two years were 80 %for stage I, 70 %for stage II, 32.1 %for stage III and 0 %for stage IV. On consideration of the low survival rates, if would seem that scanning previous to onset of symptoms and the detailed investigation of non-specific complaints, coupled with suitable surgical resection and lymph dissection could lead to longer life expectancy.
