Özet
Amaç: Alt konka lüpertrofisinin sebep olduğu burun tıkanıklarında bugüne kadar yapılmış olan medikal tedavilerin etkinliği yeterli olamamıştır. Cerrahi tedavi yöntemleri ise ağrı, kanama, kabuklanma, kuruluk, adezyon gibi morbiditelere sebep olmaktadır. Bu çalışmada alt konka hipertrolilerinde radyofrekans uygulamasının etkinliği ve güvenilirliği araştırıldı. Çalışma düzeni: Vizüel analog skorlama ile prospektif klinik çalışma Gereç ve yöntem: Kliniğimizde Ekim 2004-Şubat 2005 tarihleri arası 20 hastada medikal tedaviye dirençli alt konka hiperlrofisi tedavisinde radyolrekans uygulamalarımızda elde elliğimiz sonuçları ve morbiditeyi prospektif olarak araştırdık. Sonuçlar 8 hafta sonunda klinik muayene, hastaların preoperatif ve postoperatif şikayetlerine ve visual analog skalaya göre değerlendirildi. Bulgular: Postoperalif semptom skorlamada ise 20 hastanın l6'sında(%80) burun tıkanıklığı şikayeti hiç yoktu. 2'sinde(%IO) daha iyi bir burun solunumu vardı. 2'sinde(%lO) belirgin bir iyileşme yoktu. Hiçbiri daha kötü olduğunu ifade etmedi. Operasyondan memnun kalmayanların paranasal sinus (PNS) BT incelemelerinde konkal kemiğin konveks ve büyük olduğu izlendi. Hiçbir hastada kabuklanma, kuruluk, kanama ve postoperatif ağrı şikayeti yoklu. Sonuç: Medikal tedaviye dirençli alt konka hipertrolilerinde radyolrekans uygulaması uygulaması kolay, morbiditesi az, etkin bir tedavi yöntemi olarak değerlendirildi.
Abstract
Objective: The success of the medical treatment in turbinate hypertrophy is controversial and not fully effective. Surgical treatment methods causes morbidity such as pain, postoperative bleeding, crusting, dryness of the nose and adhesions.The efficacy and realablity of radiofrequency use in the turbinate hyperthophy cases are investigated. Study design: Prospective study with visual analog scoring Materials & methods: We evaluated the morbidity and outcomes in 20 cases of lower turbinate hypertrophy treated with radiofrequency prospectively. The outcomes are evaluated with physical examination, postoperative and preoperative complaints and visual analog scala of the patients. Results: In the postoperative symptom scoring 16 of the 20 patients (%80) did not have any nasal obstruction. 2patients (%10) had better breathing by the nose. Two patients (%10) had no certain improvement. None of the patients had reported worse breathing. Two patients who did not satisfied with the surgery had a convex and big turbinate bone in the postoperative paranasal sinus CT. Any of the patients had any complaints about crusting, dryness of the nose or posoperative pain. Conclusion: Radiofrequency use in lower turbinate hyperthophy resistant to medical treatment. Is easy, effective and has low morbidity.
