Oküler toksoplazmozlu vakalarda ön kamara sıvısı ve serumda antikor değerleri

Özet

Amaç: Oküler Toksoplazmoz tanısı oftalmolojik muaye­neye dayanır. Fakat sadece klinik değerlendirme tanıda bazen zorluklara yol açabilir. Çalışmamızın amacı oküler Toksoplazmoz vakalarında, ön kamara sıvısı ve serum antikor seviyelerinin diagnostik değerini tespit etmekti. Çalışma düzeni: Relrospektif klinik çalışma Gereç ve yöntem: Klinik olarak Toksoplazmoz düşünülen 50 hasta ile posterior üveit veya panuveitis (kontrol grubu) tanılı 25 hastada, ön kamara sıvısı ve serumda Toksoplaznıa gondii antikoru incelendi. Bulgular: Toksoplazmoz-Rctinokoroidit düşünülen 49 hastada inlraoküler antikor sente/i ispatlandı. Kontrol grubunda ise 2 hastada lokal antikor üretimi görüldü. Oküler Toksoplazmozlu 50 hastadan 49'u yüksek serum IgG-antikor tilresi gösterdi, 47'si ise IgM negatifti. Kontrol grubunda 21 hastada serum IgG yüksek, fakat IgM negatifti. Sonuç: Lokal antikor tespiti Toksoplazmoz-Relinokoroidit tanısında güvenilir bir metoddur. Serum analizi ise fa/la anlamlı değildir.

Abstract

Objective: The diagnosis of ocular toxoplasmosis is mainly-based on ophlhalmological examination but might be diffi­cult in some cases. The purpose of our study was to evalu­ate the value of aqueous humor and serum analysis in ocu­lar toxoplasmosis. Study design: Retrospective clinical trial Materials & methods: Specific IgG in aqueous humor and serum samples from 50 patients with toxoplasmic retinochoroiditis, with 25 patients with uveitis posterior or panuveitis serving as controls. Results: Specific intraocular antibody synthesis could be confirmed in 49 patients (98%). In two patients (8%) of the control group, antibody synthesis was detected. Forty-nine patients with diagnosis of ocular Toxoplasmosis were po­sitive for serum anti-T.gondii IgG, but only three patients had increased IgM levels. Conclusion: Analysis of local antibody production is a reliable method for confirming or excluding a suspected clinical diagnosis of toxoplasma retinochoroiditis. The determination of toxoplasma antibodies in the patients'serum is of limited value.
