Özet

Amaç: Bu çalışmanın amacı klinikte yatan hastalarımızda trombositoz sıklığı ve etyolojisinin araştırılması idi. Çalışma düzeni: Kesitsel çalışma Gereç ve Yöntem: Bezm-i Alem Valide Sultan Vakıf Gureba Eğitim Hastanesi İç Hastalıkları Kliniğinde 4 yıl boyunca yatırılarak tedavi edilen 2065 hastalar trombositoz varlığı ve etyolojisi açısından geriye dönük olarak araştırıldı. Bulgular: Değerlendirilen toplam 2065 hastadan 150'sinde (%7.3) trombosit sayısının normalden fazla olduğu tesbit edildi. Vakaların %63.3'ünü teşkil eden 95 hastada trombosit sayısı 401.000 - 500.000/mm³ arası, %30.7'sini teþkil eden 46 hastada 501.000 - 750.000/mm³ arası, %6'sını teşkil eden 9 vakada ise 750.000/mm³'den fazla idi. Dokuzyüzelliyedi kadın hastanın 83'ünde (%8.7), ve 1108 erkek hastanın 67'inde (%6) trombositoz vardı. Kadınlarda trombositoz erkeklere göre anlamlı derecede daha sık idi. Trombositozlara en fazla sebep olan hastalık grubu infeksiyonlar idi. Daha sonra, demir eksikliği anemisi, kollagen doku hastalıkları, cerrahi tablolar, habis neoplaziler ve miyeloproliferatif hastalıklar gelmekteydi. Kadınlarda habis neoplaziler erkeklerden anlamlı derecede daha sıktı. Diğer hastalık gruplarında ise iki cins arasında fark yoktu. Hastalıklar tek tek değerlendirildiğinde ise, demir eksikliği anemisi, kronik obstrüktif akciğer hastalığı alevlenmesi, pnömöni, tüberküloz, karsinomlar, akut hemoraji ve akut romatizmal ateş en sık rastlanan trombositoz sebepleri idi. Trombositozun etyolojisi ile ağırlık derecesi arasında bir ilişki yoktu. Vakalardan 9'u kadın 16'sı erkek olmak üzere 25'inde (%16.7) trombositozun muhtemel etyolojisi tesbit edilemedi. Hastaların çoğunda, trombositozu düzeltmeye yönelik bir tedavi uygulanmasına gerek duyulmamıştır. Sonuç: Yatan hastalarda trombositoza oldukça sık rastlanır. Bazı hallerde spesifik tedavi gerekebilir. Tüm yatan hastalar trombositoz yönünden araştırılmalıdır.

Abstract

Objective: The goal of this study was to search the fre- quency and etiological factors of thrombocytosis in our inpatient population. Study design: Cross sectional study. Materials & Medhods: The inpatient population of Bezm-i Alem Valide Sultan Vakif Gureba Training Hospital's (VGH) Internal Medicine Clinic was searched over a-4 years period for thrombocytosis. Results: Throughout the evaluation period, 2.065 patients were hospitalized and 151 of them had an elevated platelet count. Eighty-three of 957 females (8.7%) and 67 of 1108 (6%) males had an elevated platelet count. Platelet counts in 63.3% (95 patients) of the cases were between 401.000 and 500.000/mm³ in 30.7% (46 patients) of the cases, counts were between 501.000 and 750.000/mm³; and in 6% (9 patients) of the cases, counts were more than 750.000/mm³. Infections were the most common causative disease group for thrombocytosis. Other common disease groups in order of frequency were anemias, collagen diseases, surgical cases, malign neoplasias and myeloproliferative disorders. Common individual causes of high platelet count in order of frequency were iron deficiency anemia, chronic obstructive pulmonary disease, pneumonia, tuberculosis, cancer, acute hemorrhage, and acute rheumatic fever. Thrombocytosis was more frequent among females, as were malign neoplasms as an etiological factor. There was no relationship between etiological factors and platelet levels. Possible etiology of high platelet count was not found for 39 patients (25.8%): 20 males and 19 females. For most of our cases, no platelet lowering treatment was required. Conclusion:Thrombocytosis is a relatively frequent finding in hospitalized patients. In certain conditions spesific therapies may require. All hospitalized patients should be searched for thrombocytosis.
