Özet
Amaç: Tekrarlayan alt solunum yolu enfeksiyonu bulunan çocuklarda altta yalan sebep olarak gastroözofageal re nünün (GÖR) varlığı düşünülerek 24 saatlik pH monitörizasyoııu yöntemiyle reflü sıklığı araştırıldı. Çalışma düzeni: Çalışmamız bir gözlemsel araştırma olup insidans ve sebep belirlenmesine yönelik yapılmış prospektif bir grup araştırmasıdır. Gereç ve yöntem: Son altı ay içinde 2 kez, son bir yıl içinde 3 kez pnömoni (n=9. %30) veya bronşiolit (n=21, %70) geçirmiş olan, yaşları 3ay-3yıl arasında değişen. 9'u kız (%30). 21'i erkek (%70) 30 çocuk çalışmaya alındı. GÖR tanısı için, duyarlılığı ve özgüllüğü yüksek olduğu kabul edilen 24 saatlik pH monitörizasyonu kullanıldı. Bulgular: Yirmiiki çocukta (%73) GÖR tespit edilerek reflü (+) ve reflü (-) gruplar oluşturuldu. Gruplar arasında yaş. cins. boy ve ağırlık, alt solunum yolu enfeksiyonunun tipi açısından fark yoktu. GÖR (+) ve GÖR(-) grupların birbirleriyle ve ortalama referans değerleriyle karşılaştırılmasında istatistiksel açıdan en anlamlı pil parametrelerinin 5 dakikadan uzun reflü sayısı ve rcflü indeksi ölçümleri olduğu bulundu. Sonuç: Gastroözofageal reflünün, tekrarlayan alt solunum yolu olan hastalarda yüksek oranda bulunması nedeniyle, bu çocuklarda reflü varlığının araştırılması gerektiği kanısına varıldı.
Abstract
Objective: Considering the presence of GER as the underlying cause of recurrent lower respiratory tract infection in children, we aimed lo investigate the incidence of GER by mean of the 24-hour esophageal pH monitoring. Study design: Our study is an observative investigation and it is a prospective research which has been performed to determine incidence and reasons Materials & methods: The study has been carried out on nine girls and 21 boys, a total of 30 children who were between three months and three years of age and who had had pneumonia (n-9. 30%) or bronchiolitis (n=21.70%) twice during the previous six months or three times during the previous year. Results: We made use of the 24-hour esophageal pH monitoring method, which is considered as a highly specific and sensitive for GER diagnosis. We detected GER (+) in 22 children (73%) and GER (-) in eight children. Thus, two groups were formed as matter of course. We did not find any differences between these groups according to age, sex, height, weight and the type of lower respiratory tract infection. When we compared the values of the GER (+) group to those of GER (-). we found out that the pH parameters with the highest statistical significance were the reflux numbers of more than five minutes and reflux indices. Conclusion: Due to the high incidence of GER in children with recurrent lower respiratory tract infection, we concluded that this disorder should be checked in such children.
