Preeklampsi ve eklampsilerde plasental lipid peroksidasyon hasarı ve klinikle ilişkisi

Özet

Amaç: Çalışmanın amacı, preeklamptik (hafif, şiddetli, süperimpoze), eklamptik ve normal gebelerden postpartum alınan plasental doku örneklerinde lipid peroksidasyonun göstergesi olan malondialdehid (MDA) seviyesini ve maternal-fetal sonuçlarla korelasyonunu incelemektir. Materyal ve Metot: Prospektif olarak planlanan bu çalışma preeklampsi ve eklampsi tanısı ile yatırılarak izlenen ve tedavileri yapılan 83 olgu ile sistemik hastalığı olmayan 30 kontrol olgusunu kapsamaktadır. Hasta grubunu; 15 hafif preeklampsi, 23 süperimpoze preeklampsi, 23 ağır preeklampsi, 22 eklampsili olgu oluşturmaktadır. Çalışmada olguların plasental MDA değerleri ve maternal ve fetal klinik bulgularla korelasyonu değerlendirildi. istatistiksel işlemler; SPSS for windows 11 paket programı kullanılarak gerçekleştirildi. Sonuçlar: Gruplar MDA açısından değerlendirildiğinde, en yüksek plasental MDA düzeyi 28.79 µmol/gram (10.7-230.8) proteinile eklampsi grubundaydı. MDA ile umbilikal kord pH (r=-0.322, p=0.001) ve bebek doğum ağırlığı (r=-0.471, p=0.001) arasında istatistiksel olarak negatif yönde bir korelasyon olduğu saptandı. MDA ile annenin hastanede kalış süresi değerlendirildiğinde (r=0.524, p=0.001) pozitif yönde korelasyon bulundu. Tartışma: Çalışmamızda hastalık şiddeti arttıkça plasental MDA düzeylerinde artış olmuştur. Plasental MDA düzeylerinin fetal ve maternal klinik bulgularla korelasyonu bulunmaktadır.

Abstract

Objective: The aim of this study was to evaluate the malondialdehyde (MDA) levels as an indicator of lipid peroxidation in placental tissue in patients with preeclampsia (mild, severe, superimposed), eclampsia and in normotensive pregnant women and to assess an association with maternal and perinatal outcomes. Materials and Methods: Eighty-three preeclamptic and eclamptic women and 30 control patients without systemic disease were included in the study. Study group consisted of 15 mild preeclampsia, 23 superimposed preeclampsia, 23 severe preeclampsia and 22 eclampsia patients. Plasental malondialdehyde (MDA) levels were measured. The correlation of MDA levels with the maternal and fetal outcome was assessed. Statistical analysis of the data was performed with the software package SPSS for windows 11. Results: The highest MDA was 28.79 µmol/gr (10.7-230.8) protein in the eclampsia group. Placental MDA had statistically significant correlations with cord pH (r=-0.322, p=0.001), birth weight (r=-0.471, p=0.001), and maternal hospital stay (r=0.524, p=0.001). Conclusion: In our study we found an increase in placental MDA levels with the severity of disease and there were statistically significant correlations between placental MDA and fetal-maternal clinical parameters.
