Polikistik over sendromlu olgularda iki ayrı oral kontraseptif tedavisinin klinik ve metabolik etkileri

Özet
Amaç: Bu çalışmada, siproteron asetat ve desogestrel gibi iki ayrı progesteron içeren kombine oral kontraseptiflerin hasta uyumu, hirsutizm skoru ve lipid metabolizması üzerine etkilerinin araştırılması amaçlandı. Materyal ve Metot: Kıllanma yakınmasıyla kliniğe başvuran ve polikistik over sendrom tanısı alan 60 olgu çalışma kapsamına alındı ve olgular randomize edilerek iki gruba ayrıldı. Birinci gruba 0.15 mg desogestrel+0.02 mg etinil östradiol içeren, ikinci gruba 2 mg siproteron asetat+0.035 mg etinil östradiol içeren düşük doz kombine oral kontraseptif başlandı. Olguların tedavi öncesi ve 6 ay sonraki ilaç uyumu, hirsutizm skorları ve kan lipid düzeyleri değerlendirildi ve farklılıklar istatistiksel olarak analiz edildi. Sonuçlar: Her iki ilaç hastalar tarafından iyi tolere edildi ve Grup I için hasta uyumu %93 iken, Grup II için %90 idi. Olguların altı aylık tedavi sonucunda kilo, bel/kalça oranı ve vücut kitle indekslerinde bir değişim olmadığı saptandı. Serum testosteron düzeyi ve Ferriman Gallwey hirsutizm skorunun 6 aylık tedavi sonrası her iki grupta benzer oranda düştüğü gözlendi. Her iki grupta da serum LDL ve HDL düzeyi tedavi sonrası benzer oranda yükseldi. Serum TG ve total kolesterol oranı her iki grupta da yükselirken yükselmenin Grup II olgularda Grup I olgulara göre daha fazla olduğu saptandı. Tartışma: Her iki oral kontraseptif formülü hirsutizm skoru üzerine benzer etki yaparken, serum lipidleri üzerine de benzer etki gösterebilmektedir. Ama her iki grupta da TG ve total kolesterol yükselirken, yükselmenin siproteron asetat içeren grupta daha fazla olabileceği akılda tutulmalıdır.
Abstract
Objective: The aim of the study was to evaluate the effects of cyproterone acetate and desogestrel, as a part of combined oral contraceptives, on hirsutism scores, patients' drug compliance and lipid metabolism of women with polycystic ovary syndrome. Materials and Methods: Sixteen women complain of hirsutism and diagnosed as polycystic ovary syndrome were admitted study and randomized into two groups. Group I received 0.15 mg desogestrel + 0.02 mg ethinyl estradiol , Group II received 2 mg cyproterone acetate + 0.035 mg ethinylestradiol daily. Hirsutism, serum lipids were measured before and 6 months af¬ter treatments and patient's compliance were evaluated. Results: Treatment with oral contraceptive pills was well tolerated and patients' compliance was similar for two drugs (comp¬liance, 93%, 90%) respectively. No changes in weight, body mass index and waist /hip ratio was observed. While mean tes¬tosterone level and Ferriman Gallwey score was decreased in two groups. Serum LDL and HDL levels increased significantly during therapy. In addition, the difference was not statistically significant between two groups. Although serum triglyceride and total cholesterol level increased in two groups, the increase was higher for Group II. Conclusions: Both combined oral contraceptive formulations are comparably effective in decreasing hirsutism and andro¬gens. In addition, both of treatments had similar impacts on lipids. However treatment with the cyproterone acetate combi¬ned oral contraceptive is associated with a tendency toward increasing level of triglyceride and total cholesterol.
