Bilgisayarlı tomografide erken infarkt bulgularının risk faktörleri ve uzun dönem prognoz ile karşılaştırılması

Özet
Strok hastalarında, erken tanı, hem prognoz hem de tedavi planı açısından büyük öneme sahiptir. Bu çalışma bilgisayarlı tomografide gözlenen erken infarkt bulgularının (EİB) strok hastalarındaki görülme sıklığını, kısa ve uzun dönemdeki prognostik önemini ve risk faktörleri ile olan birlikteliğini belirlemek amacıyla yapıldı. Çalışmaya ilk 12 saatte kontrastsız bilgisayarlı beyin tomografisi (BBT) çekilen 110 hasta alındı. BBT'ler radyoloji bölümü tarafından değerlendirildi. BBT'de 47 (%43.6) hastada EİB saptanırken, 63 (%56.4) hastada EİB rastlanmadı. Bu hasta gruplarının 7 günlük (kısa dönem) ve 90 günlük (uzun dönem) prognozları takip edildi. Erken infarkt bulgusu varlığı, ölen veya günlük yaşamında başkalarına bağımlı olan hastalarda istatistiksel olarak anlamsız olmakla birlikte daha fazla bulundu (p=0.11, p=0.32). Günlük hayatında bağımsız olanlarda ise EİB daha az sıklıkla tespit edildi. Risk faktörlerinin dağılımında hipertansiyon varlığı EİB olanlarda istatistiksel olarak daha sık bulundu (p=0.03). İlk 12 saat içerisinde çekilen bu BBT'lerin çekim saatleri ile EİB görülmesi arasında anlamlı bir fark gözlenmedi (p=0.53). Erken infarkt bulguları BBT incelemelerinde kaydedildiği takdirde klinisyene gelişecek infarkt lokalizasyonunu göstererek hem prognozu tahmin etmekte hem de tedavi planlamasında yardımcı olacak bir bulgudur.
Abstract
Early diagnosis is important both for prognosis and the treatment deciding of strok. This study is designed to examine the prevalence and the diagnostic value of short and long term prognosis of patients and the coincidence of risk factors of stroke. 110 cases with non-contrast CT within 12 hours of stroke onset have been included to the study. CT images of cases were evaluated by radiology department. Early infarct signs (EIS) could be diagnosed on 47 (%43.6) CT scans and not visualized on 63 (%56.4) CT's. Short term (7 days) and long term (90 days) outcomes were followed and assessed with the Barthel scale. There was no relationship between the EIS and the group of death or dependent survivors (p=0.11, p=0,32). But EIS were detected more on this group of patients. Early infarct signs were less frequent in the group of good outcome who were not dependent on their daily life. Hypertension was significantly more frequent risc factor with cases EIS. There was no significant relationship between the timing at which CT was performed and the presence of EIS. If it is detected on CT scan, EIS may provide a simple tool in evaluating the prognosis of patient and treatment selection by allowing the prediction of subsequent infarct location.
