Gaziantep il merkezinde eritrosit glukoz-6-fosfat dehidrogenaz eksikliği, talasemi ve hemoglobinopati sıklığının belirlenmesi ve aralarındaki ilişkinin araştırılması

Özet
Glukoz-6-fosfat dehidrogenaz (G6PD), çeşitli biyosentez ve. detoksifikasyon tepkimeleri için gerekli olan NADPH'm üretildiği, heksoz monofosfat yolunun (HMY) anahtar enzimidir. Eritrositlerde G6PD eksikliği, kronik nonsferositik anemiden çeşitli ilaç ve besinler ile indüklenebilen akut hemolitik anemiye kadar değişen, bir çok klinik tablo oluşturmaktadır. Çalışma grubu, rastgele örnekleme sistemine göre, Gaziantep'te yaşayan 2-77 yaş arası 470'i kadın (%52.6), 424'ü erkek (%47.4) olmak üzere toplam 894 bireyden oluşturuldu. Bu bireyler, G6PD eksikliği, talasemi ve hemoglobinopati yönünden araştırılıp, bu hastalıkların birbiriyle ilişkisi incelendi. G6PD aktivite referans aralığı, 3.8-11.3 Ü/g Hb (30°C) olarak belirlendi.Bu referans aralığa göre, G6PD eksikliği oranı kadınlarda %1.5, erkeklerde %3.5 olup toplam %2.5 olarak bulundu. HbF bantı ile birlikte HbA2 yüksekliği ile seyreden P-talasemi taşıyıcılığı %0.2, HbF bantı olmadan HbA2 yüksekliği ile seyreden p-talasemi taşıyıcılığı %3.1 olarak belirlenirken, sadece HbF bantı görülmesi %0.6 oranında tespit edildi. Orak hücre taşıyıcı sıklığı %0.7 olarak saptanırken, %0.1 oranında varyant Hb rastlandı.Bu kan hastalıkları arasında" istatistiksel olarak anlamlı bir ilişki bulunamadı.
Abstract
Glucose 6 phosphate dehydrogenase (G6PD) is the rate limiting enzyme of the hexose monophosphate pathway (HMP) in which NADPH is produced, that is an important substance in a variety of biosynthesis and detoxification reactions. G6PD deficiency in the erythrocytes can lead to a spectrum of disorders ranging from chronic nonspherocytic anemia to acute hemolytic anemia which can be induced by various drugs and foods. The study group included 894 randomly selected people (470 females (52.6 %) and 424 males (47.4%)) who were living in Gaziantep. The ages of the people ranged from 2 to 77 years. These people were assessed for the presence of G6PD deficiency, thalassemia and hemoglobinopathy and for the interrelationship between these diseases. The determined reference range for G6PD was between 3.8 and 11.3 U/gHb (30°C). According to this reference range, the overall G6PD deficiency rate was 2.5 %, while this rate was found to be 1.5 % and 3.5 % in the female and male subjects, respectively. The P thalassemia trait which is accompanied by HbA2 in conjunction with HbF band was seen in 0.2 % while the P thalassemia trait which is accompanied by high HbA2 without HbF band was seen in 3.1 %. There was only HbF band in 0.6 %. There was sickle cell trait in 0.7 %. A variant Hb was encountered in 0.1 %. There was no statistically significant relationship between there blood disorders.
