Özet

Anterior spinal arter sendromu (ASAS) anterior spinal arterin tıkanması ve sonuçta spinal kord infarktı ile seyreden nadir bir nörolojik sendromdur. Bu raporda hapşırma sonrası ani başlayan tetrapleji ve solunum paralizisi ile acil servisimize başvuran gebe bir kadın hasta sunulmuştur. Hastanın kranial ve servikal kompüterize tomografisi normal olup servikal manyetik rezonans anjiografisinde sağ vertebral arterin ince ve düzensiz olduğu, anterior spinal arterin görülemediği saptandı. Hasta yoğun bakım ünitesine yatışından yedi ay sonra sepsis ve multi organ yetmezliği nedeniyle öldü. Sonuç olarak akut spinal kord infarktı nadir görülen nörolojik durumdur. Akut tetra veya paraplejiyle acil servise başvuran her hastada acil servis doktoru bu sendromu düşünmelidir.

Abstract

Anterior spinal artery syndrome (ASAS) is a rare neurological syndrome of the spinal cord due to the infarction caused by occlusion of the spinal part of the anterior spinal artery. In this report we describe a pregnant young woman presenting with sudden onset of tetraplegia and respiratory paralysis associated with sneezing. The patient's cranial computed tomography (CT) and cervical spine CT were normal. Cervical magnetic resonance angiography suggested that the right vertebral artery was thin and irregular and the anterior spinal artery could not be demonsrated. She died due to sepsis and multiorgan failure after seven months of mechanical respiratory support in intesive care unit. Emergency physician should consider this syndrome in every case of acute tetra-or paraplegia.
