Özet

Kronik inflamatuar bir hastalık olan astma tedavisinde en sık kullanılan antiinflamatuar ilaç kortikosteroidlerdir. Ancak kortikosteroidlerin astma patogenezinde önemli' rolü olan lökotrienler üzerine etkileri yoktur. Son yıllarda klinik kullanıma giren lökotrien antagonistleri ile astma tedavisinde olumlu sonuçlar alınmaktadır. Bu çalışma bir lökotrien reseptör antagonisti olan montelukastın astma tedavisindeki etkinliğini araştırmak amacıyla planlanmıştır. Astma bronşiale tanısı ile izlenen 60 olguya almakta oldukları idame tedaviye 3 ay süreyle montelukast (lOmg/gün) eklenmiş, ilaca başlamadan önceki son 3 ay ve ilacı kullandığı dönemdeki bulgular karşılaştırılmış, ilaca bağlı yan etkiler belirlenmiştir. 1.6 olgu çalışma dışı bırakılmış; toplam 44 astmalı olgu (hafif persistan 22, orta persistan 19, ağır 3) değerlendirilmiştir. İlaca başladıktan l ay, 2 ay ve 3 ay sonra yapılan kontrollerde ilaç öncesi döneme göre istatistiksel olarak anlamlı derecede gece ve gündüz semptomları ile beta2-agonist gereksinimi azalmış, solunum fonksiyon testi (SFT) değerleri ise [(FEV1 (%20) ve PEF (%22)] artmıştır (p

Abstract

Asthma is a chronic inflamatory disease and corticosteroids are the mostly used antiinflamatory agents in asthma treatment. However they don't have an effect -on leukotrienes which have an important role in asthma pathogenesis. İn the recent years there's an improvement in the outcomes of asthma management by the use of leukotriene antagonists in clinical practice. The present clinical trial was performed to investigate effectiveness of leukotriene receptor antagonist montelukast in asthma management. Montelukast (10mg/gun) was added to the maintenance therapy of 60 patients with chronic asthma for a duration of 3 months and the findings of last 3 months period without the drug and the period of drug use were compared. Furthermore the side effects of the drug were determined. 16 of the patients were excludedand and total 44 patients with asthma (mild persistent 22, moderate persistent 19, and severe 3) were evaluated.
