Yetişkin Kazara Koroziv Madde Alımlarında Akut Dönemde Endoskopi Yapılmalı mı?

Özet
Amaç: Acil tıp kliniğine kazara koroziv madde alımı ile başvuran hastaların demografik verileri ve kliniği doğrultusunda erken dönemde özefagogastroduedenoskopi (ÖGD) yapılma oranlarını ve gerekliliğini tartışmayı amaçladık. Gereç ve Yöntemler: Çalışmamıza acil tıp kliniğine Ocak 2008 ile Kasım 2010 tarihleri arasında kazara koroziv madde alımıyla başvuran hastalar geriye dönük araştırıldı. Özkıyım amaçlı koroziv madde içen hastalar çalışma dışı bırakıldı. Hastaların, yaş, cinsiyet, şikayetleri, acil servise koroziv madde alımı sonrası ne kadar sürede başvurduğu, başvuru şekli, alınan maddenin türü, ÖGD planı ve sonuçları değerlendirildi. Bulgular: Acil servise kazara koroziv madde alımı ile başvuran 28 hastanın 16’sı erkek, 12’si kadındı. Yaş ortalamaları 38.0±14.4 (18-79) yıl idi. Hastaların 16’sının aldığı madde alkali, 3 tanesi asidik özellikte olup diğerleri ise açıkta satılan yüzey temizlik ürünleri olduğundan içerik maddeleri ve oranları tam olarak bilinmemektedir. Hastalara gastroenteroloji konsultasyonu istendi. Konsultasyon sonucu hastaların 7’sine ÖGD planlandı. ÖGD yapılan hastaların birinde grade 2B, iki hastada grade 2A, 4 hastada ise grade 1 endoskopik bulgular saptandı. Grade 2B özefajit tanısı alan hastanın hastaneye yatışı verildi. Diğer hastalar 24 saat acil gözlem süresi bittiğinde taburcu edildi. Sonuç: Kazara koroziv madde alımlarında genelde alım miktarı az olduğundan iyi bir anamnez ve muayene sonrası tetkikler ve direkt grafilerle akut dönem perforasyon bulguları ekarte edilebiliyorsa acil gözlemde 24 saat takip sonrası hastaların erken dönemde invaziv bir işlem olan ÖGD yapılmadan taburcu edilmesi önerilebilir. Abstract
Objective: We aimed to discuss the necessity and the ratios of oesophagogastroduodenoscopy (OGD) in patients diagnosed with accidental corrosive substance ingestion with their demographic datas. Materials and Methods: We retrospectivelyresearched the data of patients admitted to the emergency department with the diagnosis of accidental corrosive substance ingestion between January 2008 and November 2010. Suicidal ingestions were excluded from the study. We evaluated the patients’ age, sex, compliance, the time between ingestion and admission, type of admission, type of ingested agent, OGD plan and the results of the OGD. Results: Twenty eight patients (16 male, 12 female) had admitted to our emergency department with complaint of corrosive substance intake. Mean age of the patients was 38.0±14.4 (18-79) years. Sixteen substance was alkali, 3 were acidic. The other substances were not identifi ed because they were bleaches and ingridients of them were not written on the boxes of the substances. Gastroenterology consultation was requested to all of the cases. After the consultation endoscopy was performed to the 7 of the patients. The results were; two patients were grade 2A, one was grade 2B, 4 were grade 1 esophagitis. Grade 2B was hopitalized and the others were discharged after 24 hours observation. Conclusion: The dispositon of the patients diagnosed with accidental corrosive substance ingestion after a detailed examination, laboratory evaluation, excluded perforation signs and concluded that 24 hours observation might be recommended because of the low amount of the corrosive ingestion.
