Özet

Adli Tıp Kurumu Morg İhtisas Dairesi'nde, 1994-2003 yılları arasında otopsisi yapılan toplam 28057 olguya ait otopsi raporları, birbiri ardı sıra ve ayırım yapılmaksızın taranarak, ölümü iş kazasına bağlı olarak meydana gelen olgulara ait raporlar ayrılmış ve olayın oluş biçimi, olguların demografik özellikleri, otopside saptanan bulgular, kimyasal analiz sonuçları, ölüm nedenleri açısından incelenmiştir. İncelen toplam 28057 olgudan 466'sı (%1.7) iş kazası sonucu ölmüştür. Olguların yıllara dağılımı açısından yapılan incelemede; en fazla olgunun 1997 yılında mevcut olduğu (n=67, %14.4) tespit edilmiştir. En fazla kaza Temmuz ayında (n=73, %15.7) meydana gelmiştir. Olguların 465'i (%99.8) erkek, 1'i (%0.2) ise kadındır. En küçüğü 13, en büyüğü 69 yaşında olup, yaş ortalaması 32.1 yıl (Std.sap=11.3) olarak tespit edilmiştir. En fazla olguya 21-30 yaş grubunda (n=147, %31.5) rastlanmıştır (Şekil 3). 0-18 yaş aralığında 44 (%9.4) olgu mevcuttur. İş kazasının meydana geldiği yer açısından; ilk sırayı inşaat almış (n=153, %32.8) bunu fabrika (n=33, %7.1) ve kaçak işyeri (n=27, %5.8) takip etmiş, 192 (%41.2) olguda yer belirtilmemiştir. İş kazasının meydana geliş şekli açısından ise; ilk sırayı yüksekten düşme (n=111, %23.8) almış, bunu elektrik çarpması (n=100, %21.4) ve göçük altında kalma (n=24, %5.1) izlemiş, 132 (%28.3) olguda ise kazanın meydana geliş şekli belirtilmemiştir. Ölüm nedenleri araştırıldığında; en fazla ölümün genel beden travması sonucu meydana geldiği (n=181, %38.8) ikinci sırayı elektrik çarpmalarının aldığı (n=87, %18.7) bunu ise kunt kafa travması sonucu ölümlerin izlediği (n=73, %15.7) tespit edilmiştir. Olguların 17'sinin (%3.6) kanında çeşitli oranlarda etil alkol (Şekil 4), 24'ünde (%5.2) %30-%79.3 arasında değişen oranlarda karboksihemoglobin (HbCO) 6'sında (%0.2), %1.1-%24.6 arasında değişen oranlarda Methemoglobin, 1'inde morfin, 1'inde ise amitriptilin bulunmuştur.

Abstract

Autopsy reports of a sum of 28057 cases autopsied in the Morgue Department of the Council of Foren¬sic Medicine between the years 1994-2003 were reviewed in a consecutive and non-discriminative manner. Reports related to occupational accidents were selected and examined regarding the manner of the inci¬dent, the demographic characteristics of the cases, autopsy findings, results of the chemical analysis, and the cause of death. 466 of the reviewed cases (1.7%) died due to an occupational accident. Considering the time series, 1997 was the year with the most cases (n=67, 14.4%). Most of the accidents happened in July (n=73, 15.7%). 465 of the cases (99.8%) were men and only 1 (0.2%) was a woman. The youngest of the cases was 13, and the oldest was 69 years old. The average age was found as 32.1 years (standard deviation=11.3). Most of the cases were in the age group of 21-30 years of age (n=147, 31.5%) (Figure 3). 44 cases (9,4%) were in the age interval between 0-18. Considering the place of occupational accident, construction sites took the first place (n=153, 32.8%), followed by factories (n=33, 7.1%) and illegal workplaces (n=27, 5.8%). The place of the accident was not identified in 192 cases (41.2%). " Regarding the manner of the occupational accident, falling from height took the first place (n=111, 23.8%), followed by electric shock (n=100, 21.4%) and burried under caved in (n=24, 5.1 %). The manner of the incident was not stated in 132 of the cases (28.3%). Considering the cause of death, most of the deaths occurred due to general body trauma (n=181, 38.8%), electrocution took the second place (n=87, 18.7%), followed by deaths due to blunt cranial trauma (n=73, 15.7%). Methyl alcohol in various degrees was found in the blood of 17 of the cases (3.6%) (Figure 4), carboxyl hemoglobin (HbCO) varying from 30% to 79, 3% in 24 of the cases, met hemoglobin varying from 1, 1 % to 24, 6% in 6 cases, morphine in one case, and amitriptiline in one case.
