Kuru Göz Hastalarında Üst Kapak Blefaroplasti Sonuçlarının Değerlendirilmesi

Özet

Amaç: Kuru göz hastalarında üst kapak blefaroplasti sonuçlarını, kuru göz hastalığı olmayan kontrol blefaroplasti grubu ile karşılaştırılmak. Gereç ve Yöntem: Blefaroplasti uygulanan hastalar, kuru gözü hastalığı olanlar Grup 1’i, kuru göz hastalığı olmayanlar Grup 2’yi oluşturdular. Çalışmaya, sadece cilt eksizyonunun yapıldığı üst kapak blefaroplasti uygulanan hastalar dahil edilmiştir. Her bir grupta bulunan hastaların preoperatif, postoperatif birinci hafta, birinci, ikinci ve üçüncü aylarda rutin oftalmolojik muayeneleri yapıldı. Her muayenelerinde Schirmer testi, gözyaşı kırılma zamanı değerlendirildi ve interpalpebral alan ölçümleri yapıldı. Bulgular: Çalışmaya 25 hastanın 50 gözü dahil edilmiştir. Hastaların 11’i (%44) grup 1’i, oluştururken, diğer 14 hasta (%56) ise kontrol grubu olan grup 2’yi oluşturdular. Grup 1’de 5 hasta (%45,5) Sjögren sendromu tanısıyla, 6 (%54,5) hasta ise kuru göz hastalığı nedeniyle takip edilmekteydi. Grup 1’de yaş ortalaması 51,2±7 iken, Grup 2’de 51,6±6,9 yıl idi. Her iki grup ve her iki göz için Schirmer testi sonuçları karşılaştırıldığında, hem gruplar arasında hem de grup içinde istatistiksel olarak farklı saptanmıştır (p<0,001). İki grupta da Schirmer sonucunun postoperatif birinci haftada en düşük ve üçüncü ayda en yüksek olduğu görülmüştür. Gözyaşı kırılma zamanı ve interpalpebral alan ölçümleri bakımından hem iki grupta hem de sağ-sol göz arasında farklılık saptanmamıştır. Ancak grup içinde zamana bağlı değişiklikler bakımından farklılık saptanmıştır. Her iki grupta da ölçüm değerleri, başlangıç seviyelerine ancak ikinci ayda ulaştığı görülmüştür. Sonuç: Schirmer testi, gözyaşı kırılma zamanı ve alan ölçümleri postoperatif birinci hafta içinde azalarak en düşük seviyeye ulaşıp, birinci aydan sonra yavaşca artmaktadır. Blefaroplasti ameliyatı uygulanan kuru göz hastalarının postoperatif dönemde yakından takip edilmesi önemlidir.

Abstract

Objective: To compare the results of upper eyelid blepharoplasty in dry eye patients with the control group without dry eye disease. Material and Method: Patients who underwent blepharoplasty, those with dry eyes formed group 1, and patients without dry eye disease formed group 2. Only patients who underwent upper eyelid blepharoplasty, where skin incision was performed, were included in the study. Routine ophthalmological examinations were performed in preoperative, postoperative first week, first, second and third month of patients in each group. In each examination, Schirmer test, on tear film breakup time were evaluated and interpalpebral area measurements were made. Results: 50 eyes of 25 patients were included in the study. Eleven (44%) of the patients were group 1 and the other 14 (56%) were the control group. In group 1, five patients (45.5%) were with Sjogren syndrome and the other six patients (54.5%) for dry eye disease. The mean age was 51.2±7 years in Group 1 and 51.6 ±6.9 years in Group 2. When comparing the results of the Schirmer test for both groups and eyes, it was statistically different between groups and within the group (p<0.001). When compared the Schirmer test results for both groups and eyes, there was statistically different between and within the groups (p<0.001). In both groups, the lowest Schirmer result was observed in the postoperative 1st week and the highest in the 3rd month. Tear film breakup time and interpalpebral area measurements did not differ between the two groups and in terms of the right-left eye. However, it was found to be different in terms of time-dependent changes within the group. Baseline levels were reached in the second month in both groups. Result: Schirmer test, tear film breakup time and interpaplebral area measurements decrease postoperatively in the first week and reach the lowest level. Then it increases slowly after the first month. It is important to closely monitor dry eye patients who underwent blepharoplasty surgery in the postoperative period to avoid complications.
