Özet

Malabsorbsiyon bir ya da daha fazla besin maddesinin sindirim ve emiliminin bozuk olmasıdır. Gıdaların normal sindirim ve emilimi intraluminal faz, intestinal faz ve lenfatik transport fazı olmak üzere birbirini takip eden üç fazda gerçekleşir. Bu fazların herhangi birindeki bozukluk malabsorbsiyonun ortaya çıkmasına neden olur.

Abstract

Malabsorption is defective digestion and absorption of one or more nutrients. Nutrients are normally digested in three consecutive phases called as intraluminal, intestinal and lymphatic transport. Any derangement in these phases results in malabsorption. A 59 year old female patient presented to our out-patient clinic with the complaints of lassitude,swelling of the legs and weight loss. A blind loop caused by a jejunotransversostomy performed 30 years ago with an obscure etiology was found to be the cause of malabsorption. Each step of the digestive process should be investigated in the differential diagnosis of malabsorption.T he ancillary diagnostic tests such as D-xylose test,bentiromide test, vitamin B12 absorption test and breath tests are not routinely performed. History taking, radiology imaging and laboratory methods are important in the correct diagnosis. In our patient, enzyme deficiencies specific for pancreas and stomach and diseases that may cause malabsorbtion due to lymphatic obstruction were ruled out. Considering the operation performed some 30 years ago we detected a blind loop caused by ajejunotransversoanastomosis both radiologically and endoscopically resulting in a short Bowel syndrome.
