Pediyatrik Keratoplastilerin Klinik ve Demografik Özellikleri

Özet
Amaç: Pediyatrik olgularda yapılan keratoplastilerin klinik ve demografik özelliklerini değerlendirmek ve greft sağkalımını etkileyebilecek faktörleri belirlemek. Gereç ve Yöntem: 0-18 yaş arası, derin anterior lameller keratoplasti, desme membran endotelyal keratoplasti, penetran keratoplasti cerrahisi geçiren ve en az 6 ay takibi olan olgular çalışmaya alındı. Hastaların yaşı, takip süresi, keratoplasti tipi, keratoplasti endikasyonu, ameliyat öncesi, sonrası ve son muayenedeki en iyi düzeltilmiş görme keskinliği, donör yaşı ve donör endotel sayısı, greft çapı, son muayenedeki sütür mevcudiyeti ve greft saydamlığı kaydedildi. Cerrahi tipi, hasta yaşı, greft çapı, donör yaşı, donör endotel sayısı, son muayenedeki sütür mevcudiyeti ile greft saydamlığı arasındaki ilişki değerlendirildi. Bulgular: Çalışmaya 34 hastanın 34 gözü dahil edilip, yaş ortalaması 10,91±4,22 (3-18) yıl, ortalama takip süresi 35,12±16,67(9-68) ay idi. Yirmi iki (%64,7) hastaya penetran keratoplasti, 10(%29,4) hastaya derin anterior lameller keratoplasti, 1(%2,9) hastaya Desme mebran endotelyal keratoplasti, 1 (%2,9) hastaya re-penetran keratoplasti yapıldı. Bir (%2,9) hastaya keratit, 5 (%14,71) hastaya travmatik korneal opasite, 5(%14,71) hastaya edinsel travmatik olmayan korneal opasite, 5 (%14,71) hastaya konjenital korneal opasite, 2 (%5,9) hastaya korneal distrofi, 14 (%41,2) hastaya keratokonus, 1 (%2,9) hastaya band keratopati, 1 (%2,9) hastaya ise perforasyon sebebiyle keratoplasti yapıldı. Cerrahi tipi, hasta yaşı, greft çapı, donör yaşı, donör endotel sayısı ve son muayenedeki sütür mevcudiyeti ile greft saydamlığı arasında istatistiksel olarak anlamlı ilişki saptanmadı (hepsi için p>0,05). Sonuç: Kliniğimizde pediyatrik yaş grubunda en sık keratoplasti endikasyonu keratokonus ve korneal opasite, en sık uygulanan keratoplasti tipi derin anterior lameller keratoplasti ve penetran keratoplastidir.
Abstract
Objective: We aimed to evaluate clinical and demographic characteristics of keratoplasties performed in pediatric patients and to determine the factors affecting graft survival. Material and Method: Patients aged 0-18 years who underwent deep anterior lamellar keratoplasty, desme membrane endothelial kera toplasty, penetrating keratoplasty and were followed more than 6 months were included in the study. Patients’ age, follow-up time, keratoplasty type, keratoplasty indication, best-corrected visual acuity before, after and at last examination, donor age and endothelial number, graft diameter, presence of sutures at last examination and graft clarity were recorded. The relationship between surgery type, patient age, graft diameter, donor age, donor endothelial number, presence of suture at last examination and graft clarity were evaluated. Results: 34 eyes of 34 patients were included in the study, the mean age was 10.91±4.22(3-18) years, and the mean follow-up time was 35.12±16.67(9-68) months. Penetrating keratoplasty was performed in 22(64.7%), deep anterior lamellar keratoplasty in 10(29.4%), desme membrane endothelial keratoplasty in 1(2.9%), and re-penetrating keratoplasty in 1(2.9%) patient. Keratoplasty indication was keratitis in 1(2.9%) patient, traumatic corneal opacity in 5(14.71%), acquired non-traumatic corneal opacity in 5(14.71%), congenital corneal opacity in 5(14.71%), corneal dystrophy in 2(5.9%), keratoconus in 14(41.2%), band keratopathy in 1(2.9%), and perforation in 1 (2.9%) patient. No statistically significant correlation was found between surgical type, patient age, graft diameter, donor age, donor endothelial number, and presence of suture at final examination and graft transparency (p>0.05 for all). Conclusion: The most common keratoplasty indication in the pediatric age group in our clinic is keratoconus and corneal opacity, and the most common keratoplasty type is deep anterior lamellar keratoplasty and penetrating keratoplasty.
