Rezeksiyon uygulanmış küçük hücreli dışı akciğer kanseri olgularında mediastinal lenfatik metastazın prognostik önemi ve sağkalım

Özet
Bu çalışmada, hastanemizde ameliyat edilen küçük hücreli dışı akciğer kanseri (KHDAK) olgularında, mediastinal lenf nodu hastalığı olan (N2) olguların özellikleri, rezeksiyon sonrası sağkalım ve sağkalımı etkileyen prognostik faktörleri belirledik. 1993-2001 tarihlerinde KHDAK nedeniyle rezeksiyon yapılmış 573 olgunun kayıtları geriye dönük olarak gözden geçirildi. N2 hastalığı olan 81 olgu (%14.1) çalışma grubu olarak ele alındı. Olguların 52’sine (%64.1) mediastinoskopi yapıldı. Postoperatif 30 gün içinde ölen 5 (%6.1) olgu çalışmadan çıkarılarak, 76 olgu geriye dönük olarak değerlendirildi. Olguların 74’ü (%94.7) erkek olup, yaş ortalaması 57.5’ti (35-74). Tümör, olguların 49’unda sağ (%64.5) akciğerde lokalizeydi. Olguların 39’una (%51.3) pnömonektomi, 27’sine (%35.5) lobektomi, 10’una (%13.2) bilobektomi yapılmıştı. Tümör hücre tipi, en sık squamöz karsinom olarak belirlenmişti. Hastaların 57’sinde (%75) mediastinal tek istasyon hastalığı (tek N2) bulunmuştur. Çalışmamızda %84.2 olguda komplet rezeksiyon yapılabilmiştir. Adjuvan tedavi olguların %55’ine uygulanmıştır. Tek N2 hastalığı olan olgularda medyan sağkalım 40 ay, 3 yıllık ve 5 yıllık sağkalım sırasıyla %54, %34, multipl N2’de ise medyan sağkalım 18 ay, 2 yıllık sağkalım %11’dir (log rank; 0.04) (p<0.05). İnkomplet rezeksiyon yapılmış tek N2’li olgularda sağkalım 23 ay, 3 yıllık sağkalım oranı %42, multipl N2’li olgularda ise medyan sağkalım 6 ay, 2 yıllık sağkalım oranı %0’dır. İnkomplet rezeksiyon sağkalımı olumsuz etkileyen bir faktördür (p=0.004) (p<0.05). Hastalıklı istasyonların sağkalıma etkisi karşılaştırıldığında subkarinal (n0:7) istasyon tutulumu en kötü prognoza sahiptir (p=0.003) (p<0.05). N2 hastalıkta sağkalım üzerine yaş, cinsiyet, hücre tipi, T evresi, tümörün sağ veya sol akciğerde olması, rezeksiyon şeklinin istatistiksel açıdan anlamlı etkisi yoktur. KHDAK’de mediastinal lenf nodu invazyonu olumsuz bir prognostik faktördür. Çalışmamızda, N2 hastalıkta iyi prognoz faktörleri; komplet rezeksiyon, tek N2 varlığı, subkarinal lenf nodu haricindeki lenf nodu invazyonu olarak belirlenmiştir.
Abstract
The important prognostic factor about the intrathoracic dissemination of the disease is the level of mediastinal lymph node invaded by the tumor. We aimed to determine the properties of positive mediastinal lymph nodes, the prognostic factors affecting survival and the survival rate after resections. Between December 1, 1993 and June 3, 2001, medical records of 575 patients who underwent resection for non-small cell lung cancer (NSCLC) were retrospectively examined. Eighty-one patients (41.1%) with N2 disease was accepted as the study group. In the study group, 5 patients (6.1 %) were excluded, because of mortality within the postoperative 30 days and 76 patients were evaluated postoperatively. There were 74 men (94.7%) and 2 women (5.3%) with a mean age of 57.5 years (range, 35-74 years). Tumor was located to right lung in 49 patients (44.5%), to left lung in 27 patients (35.5%). Thirty-nine patients underwent pneumonectomy (51.3%), 27 were treated with lobectomy (35.5%), and 10 had bilobectomy (13.2%). The most frequent cell type was epidermoid cancer. Mediastinal lymph node was found to be affected in 57 of patients (75%). The complete resection was performed in 84.2% of patients. The median survival in one N2 disease patients were 40 months with 3-year and 5-year survival rates as 45% and 34% respectively. On the other hand, in multipl N2 disease median survival were 18 months with a 2-year survival rate as 11% (log rank; 0.04) (p<0.05). No statistically significant effects on survival were observed for age, sex, cell type, T stage, location of the tumor to right or left lung and the type of resection in N2 disease. Mediastinal lymph node invasion is a bad prognostic sign in NSCLC. In this study we found the good prognostic factors as the presence of one N2 disease, the complete resection and the invasion of lymph nodes out of subcarinal node.
