Özet
Amaç: Bu çalışma, üriner sistem taslarının değerlendirilmesinde, kontrastsiz abdominopelvik spiral BT ile Direkt Üriner Sistem Grafisi (DÜSG) ve Ultrasonografi'nin (US) tam değerini karşılaştırmak amacıyla planlandı. Gereç ve Yöntem: Şubat 2002-Aralık 2002 tarihleri arasında polikliniğimize başvuran ve ürolojik muayenesinde üriner sistem taş hastalığı düşünülen 35'i erkek 16'sı kadın, toplam 51 hasta çalışmaya alındı. Hastalar DÜSG, US ve spiral BT ile değerlendirildi. Bulgular: Çalışmaya alman 51 hastanın yapılan kontrastsiz spiral BT incelemesinde II olguda böbrek taşı, 20 olguda üreter taşı, 1.4 olguda diğer üriner sistem patolojileri saptandı. 9 olguda böbrek ve üreler taşı birlikte bulundu. 7 olguda normal sonuç izlendi. Çalışmamızda saptadığımız taşların 23 tanesi (%73,9) 3 mm'den büyük; 6 tanesi (%20,6) 3 mm ve altındaydı. 3 mm ve altındaki taşların 6'sı.da BT' de saptanabilirken (%J00), US'da üç tanesi (%50), DÜSG'de iki tanesi (%33) izlendi. 3 mm' den büyük taşların 23 tanesi BT de (%]()()), 7 tanesi (% 30,4) US'de, 7 tanesi (% 30,4) DÜSG'de saptandı. Sonuç: Çalışmamızda DÜSG'nin sensitivitesi %31, spesifite-si %95; US'nin sensitivitesi %34, spesifitesi %100 ve BT nin sensitivitesi %/()(), spesifitesinide %97 olarak bulduk. Avantajları ve dezavantajları göz önüne alındığında, kontrastsiz spiral BT'nin, US ve DÜSG ile tanı konulamayan vakalarda kullanılmasının uygun olduğu görülmektedir.
Abstract
The aim of our study was to compare non-contrast spiral CT, US and X-ray kidney, ureter and bladder (KUB) in the evaluation of patients with urinary tract calculi. During the study period of February 2002 to December 2002, 51 patients (35 male,16 female) with suspected renal colic or ureteral colic in the urology department of our hospital underwent non-contrast spiral CT US and X-ray KUB. On CT, 11 patients had. renal calculi, 20 patients had ureteral calculi and. 14 patients had other urinary tract pathologies. 9 patients had. renal and ureteral calculi, in the same time.7 patients were normal. 23 calculus were greater than 3 mm (% 73.9); 6 calculus were 3 mm and. smaller (%20.6) on spiral CT. All the 6 calculus which diameter 3 mm and smaller were detected on CT(%J00), only 3 of this small calculus were detected, on US (% 50) and only 2 of them detected on X-ray KUB(% 3 3). All the 23 calculus which diameter greater than 3 mm were detected on CT (%/00), 7 of them were detected, on US (% 30.4) and 7 of them detected on X -ray KUB(% 30.4). In our study, the sensitivity of X-ray KUB was %31 and. specificity was % 95; the sensitivity of US was % 34, specificity was % 100 and the sensitivity of CT was %100 and. specificity was %97. When appreciating all the advantages and disadvantages of non-contrast spiral CT, it should, be reserved for cases where US and X-ray KUB do not show the cause of symptoms.
