Özet

Amaç: Çalışmamızda, postoperatuar ağrı kontrolünde trama- doliin hasta kontrollü analjezide (HKA) kullanımında bohts doza eklenen bazal infiizyonun etkinliğini araştırmak amaç¬landı. Gereç ve yöntem: Genel anestezi altında abdominal histerek- totni geçirecek 36 olgıı standart anestezi uygulaması sonrası randomize olarak 3 gruba ayrıldı. HKA Grup I'de bolus doz, Grup IV de bol us doz + düşük doz, Grup IH'de boîus doz + yüksek doz bazal infüzyon uygulamalarıyla sağlandı. Posto¬peratuar ağrı (Verbal Rating Scale-VRS- ile), solunum hızı, sedasyon skorları operasyon sonrası 1., 2., 4., S., 12. ve 24. saatlerde kaydedildi. 24 saatlik toplam analjezik t ilkelimi he¬saplandı. Yan etkiler takip edildi. Bulgular: VRS; Grup I'de, Grup 11 ve Grup III'e göre yüksek, solunum hızı; Grup II ve Grup III'de, Grup I'e göre düşük ol¬makla birlikte, aradaki fark anlamlı değildi (p>0.05) Sedas¬yon skoru ve toplam analjezik tüketimi; Grup III'de, Grup I'e göre daha yüksek bulundu (p<0.05). Yan etkiler olarak gast¬rointestinal sisteme ait olanlar Grup II ve Grttp III'de Grup I'e göre daha sık görüldü. Sonuç: Tramadoliin postoperatuar ağrı tedavisi sağlamak amacıyla HKA'de kullanımında, bolus doza eklenen bazal in¬füzyon uygulamasının, ağrıyı kontrol etmede bolus doza göre anlamlı bir üstünlük sağlamadığı, yanısıra total analjezik tü¬ketiminde artışa neden olduğundan, başta solunum depresyo¬nu olmak üzere yan etkileri artıracak bir faktör olduğu sonu¬cuna varıldı.

Abstract

Objective: In our study ive aimed to investigate the effective¬ness of basal infusion added to bolus dose for patient control¬led analgesia (PCA) for postoperative pain management. Study design: 36 patients undergoing abdominal hysterec¬tomy under general anaesthesia were randomized to three groups after standart anaesthetic procedure. In Group I bolus dose regimen, in Group II bolus dose+low basal infusion ra¬te, in Group III bolus dose+high basal infusion rate were used for PC A. Postoperative pain evaluation(with Verbal Ra¬ting Scale-VRS-), respiration rate, sedation score were recor¬ded at L, 2., 4., 8,, 12. and 24. hours postoperatively. Total analgesic consumption for 24 hours was calculated. Side ef¬fects were observed. Results: VRS was higher in Group I than Group II and Group HI and respiration rate was lower in Group II and Group III than Group I. But these differences were not statistically sig¬nificant (p>0.05). Sedation score and total analgesic con¬sumption were significantly higher in Group III than Group I (p<0.05). Gastrointestinal side effects were more frequent in Group II and Group III than Group I. Conclusion: We concluded that basal infusion added to balus dose has not provide a significant superiority to bolus dose alone when used with PCA for postoperative pain management. Also side effects, mainly respiratory depression, can be associated with the increased total analgesic con¬sumption caused by basal infusion.
