Özet

Lupus vulgaris (LV), deri tüberkülozunun nadir görülen ve yumuşak dokularda kronik ilerleyici bir yıkım oluşturarak atrofik skaıiaşmaya yol açan bir tipidir. Hastalığın etkenleri Mycobacterium tuberculosis veya M. bovis, nadir olgularda bacillus Calmette-Guerin (BCG) atlamasıdır. Etkenin saha¬ya inokülasyonu sıklıkla hematojen, lenfatik veya başka bir tüberküloz lezyonundan direkt yayıl imla dır; çok nadiren primer inokülasyon oluşabilir (1). LV'nin en sık görülen komplikasyonlan, skar kontraktürü ve ülsere lezyonların sekonder enfeksiyonlarıdır; nadir de olsa hu lezyonlardan tümör gelişebilmektedir (yassı epitel hücreli kanser, bazal hücreli kanser, sarkom ve lenfoma) (2). Bu çalışmada, sag dizinde LV zemininde yassı epitel hücreli kanser gelişen bir olguyu sunuyoruz.

Abstract

Lupus vulgaris (LV) is a rare form of cutaneous tuberculosis producing slowly progressive destruction of the soft tissues followed by atrophic scarring. The disease is caused by Mycobacterium tuberculosis or M. bovis and, in rare cases, by bacillus Calmette-Guerin vaccination. The inoculation is usually by hematogenous, lymphatic or direct, spread from another tuberculous lesion. There is rarely a primary inoculation. The most frequently seen complications of LV are scar contracture and secondary infections of ulcerated lesions. Occasionally tumor (squamous cell carcinoma, basal cell carcinoma, sarcoma and lymphoma) can develop from these lesions. In this study, we are presenting a case of squamous cell carcinoma formation following LV on the right knee.
