Yarık damak operasyonları anestezisi güçlükleri: 1999-2004 yılları arası deneyimlerimiz

Özet
Amaç: 1999-2004 tarihleri arasında Plastik Cerrahi Kliniği tarafından yarık damak nedeni ile öpere olan 93 olgunun değerlendirilmesi amaçlandı. Gereç ve yöntem: Anestezi ve Plastik Cerrahi Klinikleri kayıtlarından retrospektif olarak, olguların operasyon yaşlan, cinsiyetleri, eşlik eden anomalileri, ameliyat, tipi, ameliyat süresi, ameliyat sırasında ve sonrasında gelişen komplikasyonlar araştırıldı. Bulgular: Öpere edilen 93 olgunun % 37'si kız, % 63'ü erkekti. Kızların yaş ortalaması 25±10 ay, erkeklerin yaş ortalaması 23±I2 ay olarak tespit edildi. 2 olguda (%2) Pierre Robin sendromu, 1 olguda (%I) Larsen Sendromu mevcuttu. Yarık damak anomalisine 2 olguda (%/) atrial septal defekt (ASD), I olguda (%J) ventral septal defekt (VSD), 1 olguda (%I) ise anal ve renal agenezi eşlik etmekte idi. 72 olguya (%77) Veau Wardill Kinler, 15 olguya Von Langenbeck (%16) ve 6 olguya (%7) Furlow yöntemleri ile damak ameliyatı uygulandığı tespit edildi. Operasyon süresi ortalama 188±25 dakika idi. 3 olgu (%3) endotrakeal entübe edilemedikleri için başka bir merkeze sevk edildi. 11 olguda (%I2) erken postoperatif solunum sıkıntısı geliştiği için postoperatif yoğun bakım ünitesinde takip edildi. 2 hastada (%2) erken postoperatif dönemde körlük geliştiği tespit edildi. Sonuç: Yarık damak operasyonları teknoloji ve ameliyat tek¬niklerindeki gelişmelere rağmen hastaların beraberinde eşlik eden hastalıkları ve gelişebilecek komplikasyonlar açısından anestezi hekimlerinin en dikkatli olması gereken vaka gruplarındandır.
Abstract
Objective: 93 cases of cleft palate, operated by department of plastic and reconstructive surgery in our hospital between 1999-2004, were evaluated. Study design: In this retrospective analysis of the patient, records, patients were evaluated according to age, gender, accompanied malformations, type and duration of operation, and peroperative and postoperative complications. Results: 37 percent of 93 patients were female while 63 percent were male. Mean age were 25±10 and 23±12 months for female and male patients, respectively. 2 of the patients (2%) were diagnosed as Pierre-Robin sequence, 1 patient (1%) as Larsen syndrome. Two of the patients (2%) were presented with atrial septal defect (ASD), one of the patients (1%) had ventricular septal defect (VSD), while 1 patient (1%) had renal and anal agenesis. 72 of the patients (77%) were used Veau Wardill Kilner technique, 15 of the patients (16%) were performed Von Langenbeck tecnique and 6 of the patients (7%) were repaired Furlow tecnique. Average of operation time was 188±25 minutes. Three of our patients (3%) could not be entubated and consequently operated. 11 of the patients (12%) experienced postoperative respiratory stress and followed up in 1CU for a limited time. Blindness devoloped in 2 of the patients (2%). Conclusion: In spite of the improving technologies and growing experience in anesthesiology and plastic surgery, cleft palate patients remains as a challenging task due to specific comorbid conditions and postoperative complications.
