Tonsillektomi operasyonlarında remifentanil ve alfentanilin hemodinami ve derlenme yönünden karşılaştırılması

Özet

Amaç: Tonsillektomi operasyonlarında remifentanil ve alfentanilin peroperatif hemodinami ve postoperatif derlenme yönünden karşılaştırılmasını amaçladık. Gereç ve yöntem: Tonsillektomi operasyonu planlanan 30 çocuk olgu rastgele iki gruba ayrıldı. Olgulara aynı anesteziklerle indüksiyon yapıldı. Grup R'ye 1 mcglkg bolus ardından 0,5 mcglkgldk infüzyon dozunda remifentanil, Grup A'ya 10 mcglkg bolus ardından 2,5 mcglkgldk infüzyon dozunda alfentanil verildi. Olguların anestezi, cerrahi süreleri ve ekstübasyon zamanlan kaydedildi. Başlangıç, entübasyon, ağız açacağı öncesi ve sonrası, tonsillektomi insizyonu öncesi ve sonrası ve ekstübasyondaki Sistolik Kan Basıncı(SAB), Diastolik Kan Basıncı(DAB), Kalp Atım Hızı (KAH) ve Oksijen Satürasyonu (SpO2) değerleri işaretlendi. Ekstübasyon sonrası hemodinamik değerleri, SpO2, PACU taburcu skorları ve CHEOPS ağrı değerleri kaydedildi. Bulgular: Entübasyon sırasındaki KAH Grup A'da anlamlı yüksek bulundu. Grup R'de ağız açacağı öncesi ve sonrası, tonsillektomi insizyonu öncesi ve sonrası SAB değerleri anlamlı düşük bulundu. Ekstübasyon süresi Grup A'da anlamlı uzundu. Grup A' da postoperatif SpO2 değerlerinde anlamlı düşüşler saptandı. PACU taburcu skorlarının 1.dakika değerinin Grup R'de anlamlı yüksek olduğu görüldü, (p <0.05) Sonuç: Pediatrik tonsillektomilerde remifentanilin alfentanile göre daha iyi hemodinamik stabilite sağlayacağı, kısa sürede ekstübasyona izin vereceği, postoperatif dönemde daha az hipoksiye yol açacağı düşüncesindeyiz.

Abstract

peroperative hemodynamic parameters and postoperative recovery effects of remifentanil and alfentanil on patients undergoing tonsillectomy. Study design: 30 pediatric patients were randomly divided into two groups. All of the patients received the same anesthetics agents during induction. Group R had 1 mcglkg bolus then 0,5 mcglkglmin infusion dose of remifentanil. Group A had 10 mcglkg bolus then 2$ mcglkglmin infusion dose of alfentanil. Anesthesia, surgery and extubation times of each patient were registered. At the begining, during intubation, before and after installation of mouth-opening device, before and after tonsillectomy incision and during extubation SBP, DBP, HR and SpO2 values were noted. After extubation hemodynamic parameters, PACU discharge scores and CHEOPS pain scores were registered. Results: In Group A, HR was found significantly higher during intubation. In group R, before and after installation of mouth-opening device, before and after tonsillectomy incision the SBP values were found significantly lower. Extubation time was significantly longer in Group A. In Group A, postoperative SpO2 values were significantly low. The PACU scores of Group R at the first minute was high (p <0.05). Conclusion: Our study shows that in pediatric tonsillec-tomies in comparison with alfentanil, remifentanil gives better hemodynamic stability, allows earlier extubation and causes less hypoxia during postoperative period.
