Özet
Amaç: Pilonidal sinüsün tedavisinde cerrahinin temel metod olduğu bilinmektedir. Taksim Hastanesi I. Genel Cerrahi Kliniği olarak 1998-2000 yılları arasında ameliyat ettiğimiz 56 pilonidal sinüs olgusunu literatür bilgileri ile destekleyerek sunmak istedik. Yöntem:Olgularımızı değişik cerrahi tekniklerle ameliyat ettik. Olgularımızın 15'ine(%26.78) kist eksizyonunu takiben primer kapama yaptık. 30 olguya(%53.57); kist eksizyonu ve marsupializasyon uyguladık, 1 olguya(%1.78) oblik eksizyon, 10 olguya ise (%17.85) kist eksizyonunu takiben rhomboid flap ile onarım yaptık. Hastalarımızı 6 ay ile 1 sene arasında takip ve kontrol ettik. Sonuçları toplu halde sunduk. Bulgular : Olgularımızın 53'ü(%94.64) primer; 3 tanesi (%5.36) ise nükstü. Olgularımız 17-44 yaşları arasında olup ortalama yaş 20 idi. 56 olgunun 11'i (%19.64) kadın; 45'i (%80.36) erkekti. Olguların 15'ine(%26.78) kist eksizyonu ve primer kapama operasyonu yapıldı. Bu 15 olgunun (%26.78) 2 tanesinin (%13.33) ortalama 6 aylık takiplerde nüks ettiği görüldü. Kistin eksizyonunu takiben marsupyalizasyon yapılan 30 olgunun (%53.57); 2 tanesinde (%6.66) yara iyileşmesi gecikti. Oblik eksizyon yapılan 1 olguda (%1.78) komplikasyon gelişmedi. Kist eksizyonunu takiben Rhomboid flap ile onarılan 10 olgunun (%17.85) ise hiçbirinde komplikasyon gelişmedi. Sonuç:Basit pilonidal kistlerde eksizyon ve primer kapama yöntemi kullanılabilir ama bölgenin kıldan arındırılması çok önemlidir. Kronik ve nüks olgularda ise; eksizyon ve flap ile onarım teknikleri ile (örneğin Rhomboid flap kaydırma) iyi neticeler alınabilmektedir. Komplike olgularda kistin eksizyonu ve marsupyalizasyonu ile elde edilen sonuçlar yüz güldürücüdür ama hastayı uzun süreli pansuman ihtiyacına maruz bırakır.
Abstract
OBJECTIVE: Surgery is known to be the principal method in treatment of pilonidal sinus. We intended to present our 56 pilonidal sinus cases which have been operated in Taksim Teaching and Research Hospital I. General Surgery Clinic between 1998-2000 , our results were supported by the literature. METHOD: Operations were performed with various surgical technique 15 patients (%26.78) underwent primary closure following cyst excision; 30 patients(%53.57) marsupyalisation following) cyst excision and 1 patient (%1.78) oblique excision; 10 cases (%17.85) rhomboid flap following cyst excision. Follow up period was 6 months to 1 year and results were presented. RESULTS: Of the cases , 53(%94.64) were primary and 3(%5.36) were recurrent. Ages of our cases ranged between 17 and 44 , the mean age was 20. Of the 56 cases 11(%19.64) were female and 45 (%80.36) were male. 15 patients(%26.78) underwent cyst excision and primary closure opeation. During 6 months follow up period, 2(%13.33) of this 15 (%26.78) patients had recurrence. In 2(%6.66) of the 30(%53.57) patients who had marsupyalisation after cyst excision , wound healing delayed. In one patient who had oblique excision (% 1.78), no complications were seen. None of the 10 patients (%17.85) who had rhomboid flap after cyst excision had complications. CONCLUSION: Excision and primary closure method can be used in simple pilonidal cysts but eradication of the area from hair is essential. In chronic and recurrent cases, good results can be taken with excision and flap slipping techniques. In complicated cases, results of cyst excision and marsupyalisation are satisfactory but exposes patient to long term wound care.
