BİR ÜNİVERSİTE HASTANESİNDE MYCOPLASMA HOMINIS VE UREAPLASMA UREALYTICUM PREVALANSI VE ANTİBİYOTİK DİRENÇ PROFİLİ

The Prevalence and Antibiotic Resistance Profiles of Mycoplasma hominis and Ureaplasma urealyticum at a University Hospital

Özet

Ürogenital sistemden en sık izole edilen mikoplazmalar Mycoplasma hominis ve Ureaplasma urealyticumdur. Çalışmamızın amacı vajinit ön tanılı kadın hastalarda genital U.urealyticum ve M.hominis sıklığının ve antibiyotik direnç prof ilinin belirlenmesidir. Çalışmaya Turgut Özal Üniversitesi Tıp Fakültesi Hastanesi Mikrobiyoloji Laboratuvarına Nisan 2011-Mayıs 2014 tarihleri arasın- da Kadın Hastalıkları ve Doğum Polikliniğine başvuran vajinit ön tanılı 4391 hastadan alınan vajen örnekleri dahil edilmiştir. Örnekler eküvyon ile alınarak steril taşıma besiyeri (Copan, İtalya) içinde laboratuvara gönderilmiştir. Mycoplasma IST 2 test (BioMérieux, Fransa) kiti kullanılarak M.hominis ve U.urealyticum varlığı ve üreme saptanan örneklerde etkenlerin doksisiklin, of loksasin, eritromisin, tetrasiklin, siprof loksasin, azitromisin, klaritromisin, josamisin ve pristinamisine duyarlılıkları belirlenmiştir. Örneklerin 3686ında üreme olmazken, 558sinde U.urealyticum ve 147‘sinde M.hominis izole edilmiştir. U.urealyticum suşlarında siprof loksasine % 75.8, of loksasine % 47.8, eritromisine % 22, klaritromisine % 19.7, azitromisine % 15.6, josamisine % 8.4, pristinamisine % 6.5, tetrasikline % 3.2, doksisikline % 2 direnç saptanmıştır. M.hominis suşlarında ise eritromisine % 95.9, klaritromisine % 89.1, siprof loksasine % 87.1, of loksasine % 87.1, azitro- misine % 83, josamisine % 25.8, pristinamisine % 15, tetrasikline % 12.3, doksisikline % 6.8 direnç bulunmuştur. Bu sonuçlar değerlendirildiğinde her iki mikroorganizmanın duyarlılığın en yüksek olduğu antimikrobiyaller sırasıyla doksisiklin, tetrasiklin, pristinamisin ve josamisin olarak saptanmıştır. Tetrasiklin grubu antibiyotiklere karşı duyarlılık yüksek olmasına rağmen tedavi sonrası oluşabilecek oluşabilecek yan etkiler göz önünde bulundurulmalıdır. Özellikle M.hoministe makrolid ve kinolon grubu antibiyotiklere direncin yüksek olması tedavide bu grup antibiyotiklerin kullanımını kısıtlamaktadır. Sonuç olarak M.hominis ve U.urealyticumun etken olduğu düşünülen inf eksiyonlarda yapılan antibiyogram test sonucuna göre uygun tedavinin verilmesi önem taşımaktadır.

Abstract

The most f requently isolated mycoplasmas f rom the urogenital tract are Ureaplasma urealyticum and Mycoplasma hominis. The aim of this study was to determine the prevalance and antibiotic resistance prof iles of genital U.urealyticum and M.hominis in women with a prediagnosis of vaginitis. Vaginal specimens that were sent to the microbiology laboratory of Turgut Özal University Hospital f rom the gynecology and obstet- rics outpatient clinic f rom 4391 patients with a prediagnosis of vaginitis between April 2011 and May 2014 were included in this study. The samples were taken using sterile swab tubes with transport medium (Copan, Italy). The presence of M.hominis and U.urealyticum and the antimicrobial susceptibilities aganist doxycycline, of loxacine, erythromycin, tetracycline, ciprof loxacin, azithromycin, clarithromycin, josamy- cin and pristinamycin were determined using Mycoplasma IST 2 test (BioMérieux, France). While there wasn’t any growth in 3686 of patients, U.urealyticum was detected in 558 and, M.hominis was detected in 147 patients. The resistance rates of U.urealyticum were as f ollows: Ciprof loxacin 75.8 %, of loxacin 47.8 %, erythromycin 22 %, clarithromycin 19.7 %, azithromycin 15.6 %, josamycin 8.4 %, pristi- namycin 6.5 %, tetracycline 3.2 %, doxycycline 2 %. The resistance rates of M.hominis were as f ollows: Erythromycin 95.9 %, clarithromy- cin 89.1 %, ciprof loxacin 87.1 %, of loxacin 87.1 %, azithromycin 83 %, josamycin 25.8 %, pristinamycin 15 %, tetracycline 12.3 %, doxyc- ycline 6.8 %. Considering these results, highest rates of susceptibility of both microorganisms were to doxycycline, tetracycline, pristinamycin and josamycin, respectively. Although the activity of tetracycline antibiotics is high, potential side eff ects that may occur af ter treatment should be considered. Because the resistance rates of macrolide and quinolone antibiotics were high especially f or M.hominis, use of these is restricted in the treatment. In conclusion, administration of appropriate treatment f or inf ections caused by U.urealyticum and M.hominis according to results of antimicrobial testing is important.
