Özet

Son dönem böbrek yetmezliğinin en önemli ve başarılı tedavisi olan renal transplantasyon sonrasında başta üriner sistemde olmak üzere infeksiyonlar önemli sıklıkta görülmektedirler. Fakültemizde renal transplantasyon yapılmış hastalardaki üriner sistem infeksiyonu sıklığı, etkenleri ve risk faktörlerinin tespit edilmesi amaçlanmıştır. Çalışmada 63 hasta bir yıllık prospektif takibe alınmıştır. Risk faktörlerinin değerlendirilmesi amacıyla kronik böbrek yetmezliği etiyolojileri, preoperatif diyalize girme süreleri, operasyon süreleri, canlı veya kadavradan nakil yapılması, dren ve kateterle takip süreleri, aldıkları immün baskılayıcılar, hastanede yatış süreleri ve sayıları gibi değişkenler takip edilerek hastalarda görülen üriner sistem infeksiyonlarıyla ilişkileri değerlendirilmiştir. Oniki hastada toplam 24 infeksiyon atağı görülmüş, en sık izole edilen mikroorganizmalar Escherichia coli ve Klebsiella pneumoniae olarak belirlenmiştir. İnfeksiyonların en sık görüldüğü zaman aralığı transplantasyon sonrası ilk 120 gün olarak tespit edilmiştir. İncelenen değişkenler arasında nakil sonrası taburcu edildikten sonraki hastaneye yatış sayılarının, infeksiyon atağı geçiren hastalarda, infeksiyon atağı geçirmeyenlere göre istatistiksel olarak anlamlı olmasa da (p=0.052) belirgin derecede fazla olduğu görülmüştür. Bunun dışındaki değişkenler açısından belirgin farklılık saptanmamıştır. Renal transplantasyon yapılan hastaların takibinde ilk bir yıl, özellikle ilk 120 gün içinde üriner sistem infeksiyonları açısından dikkatli olunmalıdır. Çeşitli nedenlerle hastaneye yatırılan nakil hastaları, özellikle de transplantasyon sonrası üriner sistem infeksiyonu geçirme öyküsü var ise, üriner infeksiyon gelişimi açısından mutlaka değerlendirilmelidirler. *25.ANKEM Antibiyotik ve Kemoterapi Kongresi’nde sunulmuştur.

Abstract

Renal transplantation, the most important and successful treatment of end stage renal failure, usually results with infections, especially with the urinary tract infections.We wanted to determine the infection ingredients and risk factors of urinary tract infections after renal transplantation at the patients in our hospital. Sixtythree patients were evaluated for 1 year prospectively. The risk factors, variables such as the etiology of chronic renal failure, the duration of preoperative dialysis, operative duration, transplantation from alive or deceased donor, follow up length of the drains or catheters, used immune suppressives, time and number of hospitalization were examined and the relationship between the risk factors and urinary tract infections were evaluated. Totally 24 infection attacks were observed in 12 patients. Escherichia coli and Klebsiella pneumoniae were found to be the most isolated microorganisms. The infections after transplantation frequently were observed within 120 days. For the other variables, there was no significant difference. Assessing all the variables, hospitalization numbers of the transplant patients after the postoperative discharge were detected significantly more frequent in patients with urinary tract infection attacks as compared with the transplant patients without infection attacks, but this difference was statistically insignificant (p=0.052). At the first year of renal transplantation followup, especially the first 120 days, the attention must be paid for urinary tract infections. The transplant patients hospitalized due to different reasons, must be evaluated from the view of urinary tract infection development, especially if they had history of urinary tract infection in post-transplant period.
