İnvazif aspergilioz kuşkulu hastalarda galaktomannan antijen testi sonuçlarının değerlendirilmesi

Özet
Bu çalışmada, olası veya şüpheli invazif aspergilloz (İA)’lu hastalarda galaktomannan (GM) antijen testinin hastanemizdeki sonuçlarının değerlendirilmesi amaçlanmıştır. Hastanemiz Hematoloji-Onkoloji kliniklerinde takip edilen nötropenik ateşli hastaların serum örneğinde GM antijeni ELİSA yöntemi ile incelenmiş ve sonuçlar retrospektif olarak değerlendirilmiştir. EORTC/MSG tanım kriterlerine göre hastalar; kanıtlanmış, olası ve şüpheli invazif fungal infeksiyonu (İFİ) olanlar şeklinde gruplandırılmıştır. Toplam 48 nötropenik ateşli hastadan alınan 181 serum örneğindeki GM antijeni sonuçları değerlendirilmiştir. Ardışık alınan 169 serum örneğinin 31’inde (% 18.3) GM pozitif saptanmıştır. Tek serum örneği çalışan 12 hastada ise GM negatif bulunmuştur. Olgular içerisinde kanıtlanmış İFİ olgusu saptanmamıştır. Şüpheli akciğer lezyonu olan 12 hastanın 10’unda aynı zamanda GM pozitifliği saptanmış ve olası İFİ, diğer iki hastada ise GM negatif saptanmış ve şüpheli İFİ olgusu olarak kabul edilmiştir. GM testi ile daha sağlıklı sonuçlar elde edebilmek için olguların İA açısından histopatolojik ve kültür ile kanıtlanmasının sağlanması ve yalancı pozitifliklere neden olacak etkenlerin gözönünde tutulması gerektiği kanısına varılmıştır.
Abstract
In this study, the evaluation of galactomannan (GM) antigen test results of patients with probable or possible invasive aspergillosis (IA) in our hospital was aimed. GM antigen was determined by ELISA method in serum samples of febrile neutropenic patients followed in Haemotology-Oncology Units of our hospital and the results were evaluated retrospectively. The patients were grouped as proven, probable and possible invasive fungal infections (IFI) according to EORTC/MSG definition criteria. The results of GM tests in 181 serum samples of totally 48 febrile neutropenic patients were analysed. GM antigen was positive in 31 of 169 consecutive serum samples taken. GM antijen was found as negative in 12 patients whom serum samples were taken once. Proven IFI case could not be determined among all cases. In ten of 12 patients with suspected lung lesions, GM was also positive and these patients were considered as probable IFI, in other two patients GM was negative and these were considered as possible IFI patients. In order to get more accurate results with GM test, the diagnosis of IA should be confirmed by histopathologic and microbiologic investigation and the factors causing false positivity of the test should also be considered.
