Genotip 1 ile infekte kronik hepatit c hastalarında peginterferon ve ribavirin tedavisinin etkinliği: düşük ve yüksek viral yük etkisinin karşılaştırılması

Özet
Kronik hepatit C (KHC) infeksiyonunun tedavisi halen peginterferon alfa ve ribavirin kombinasyonudur. Hepatit C virüs (HCV) genotip 1’in diğer viral genotiplere göre tedavisi daha zordur. Bu çalışmada HCV genotip 1 ile infekte hastalarda peginterferon alfa-2b ve ribavirin tedavisinin etkinliği araştırılmıştır. Genotip 1 ile infekte 50 naiv KHC hastasına peginterferon alfa-2b 1.5 μg/kg/hafta ve vücut ağırlığına orantılı ribavirin 1000-1200 mg/gün tedavisi uygulanmıştır. Tedavi sonucunda amaç, kalıcı viral yanıt (KVY), yani tedavi bitiminden 24 hafta sonra duyarlı PCR yöntemi ile serumda HCV-RNA saptanmaması olarak belirlenmiştir. Toplam 50 hastanın 30’unda (% 60) KVY elde edilmiştir. Serum HCV viral yükü, tedavi başlangıcında hastaların 27’sinde > 500,000 İU/ml ve 23’ünde < 500,000 İU/ml olarak belirlenmiştir. KVY oranı, viral yükü düşük hasta grubunda (% 70), viral yükü yüksek hasta grubuna (% 52) göre daha yüksek olarak saptanmasına rağmen bu fark istatistiksel olarak anlamlı bulunmamıştır. Genel literatüre göre rölatif olarak yüksek saptanan KVY oranının, hasta grubumuzun tedaviye uyumunun yüksek olmasına ve karaciğer fibroz skoru düşük hastaların oranının yüksekliğine bağlı olabileceği düşünülmüştür. Ayrıca ırk ve etnik farklılıkların da bunda rol oynayabileceği düşünülebilir.
Abstract
Chronic hepatitis C (CHC) is optimally treated with a peginterferon alfa and ribavirin combination therapy. Hepatitis C virus (HCV) genotype 1 infections are more difficult to treat than the infections with other viral genotypes. This study investigates the efficiency of peginterferon alfa-2b and ribavirin treatment in Turkish CHC patients infected with HCV genotype 1. Fifty treatment-naive CHC patients infected with genotype 1 were treated with peginterferon alfa-2b 1.5 μg/kg/week and weight-based ribavirin 1000-1200 mg/day. The main goal of the therapy was to attain a sustained virological response (SVR), defined as the absence of HCV-RNA from serum by a sensitive PCR assay, after 24 weeks following discontinuation of therapy. SVR was achieved in 30 (60 %) of 50 patients. In 27 of the patients, serum HCV viral load was > 500,000 IU/ml and in 23 was < 500,000 IU/ml at the begining of the therapy. Although did not reach to statistical significance, SVR rate was higher in patients in the low viral load group (70 %) than in the high viral load group (52 %). Relatively high rate of SVR is explained by good compliance and adherence to therapy of our patient group and high percentage of patients with a mild or no fibrosis in liver histology. Interestingly, race and ethnical differences might have played a role.
