Özet

Hastane kökenli santral sinir sistemi infeksiyon (HKSSSİ)’ları önemli morbidite ve mortalite nedeniyle ciddi hastane kökenli infeksiyon (HKİ)’lar arasında yer almaktadırlar. Bu çalışmada Eskişehir Yunus Emre Devlet Hastanesi’nde HKSSSİ tanısıyle izlenen olguların klinik, laboratuvar, tedavi ve tedaviye yanıt bulgularının retrospektif olarak incelenerek verilerin paylaşılması amaçlanmıştır. Çalışma süresince hastanemizde tespit edilen 648 HKİ’nun 15 (% 2.3)’i HKSSSİ olgusu olmuştur. Aynı sürede hastanemizde yatarak tedavi gören 116722 hastada HKSSSİ insidans hızı % 0.01 olarak tespit edilmiştir. HKSSSİ’da en sık izole edilen etkenler 2 metisiline dirençli koagulaz negatif stafilokok , 2 Acinetobacter spp., 1 Klebsiella spp., 1 Enterococcus faecium, 1 metisiline duyarlı Staphylococcus aureus olarak saptanmıştır. Etkenler bir olguda subdural ampiyem materyalinden, bir olguda kandan, bir olguda kan ve BOS’dan, dört olguda BOS’dan izole edilmiştir. 2 (% 13) hastada şant revizyonu uygulanırken 3 (% 20) hasta eks olmuştur. Ampirik antibiyotik tedavisinin planlanması için daha geniş serilere ihtiyacımız olmakla birlikte çalışmanın ülkemiz ve hastanemiz açısından önemli bir veri kaynağı oluşturacağı düşünülmüştür.

Abstract

Hospital acquired central nervous system infections (HACNSI) are classiŞed as serious hospital acquired infections (HAI) due to high mortality and morbidity rate. This study aims to investigate the clinical and laboratuar Şndings and treatment response to treatment of HACNSI at Yunus Emre State Hospital. 15 (2.3 %) out of 648 HAI patients were HACNSI during the study period. The incidence of HACNSI among 116722 hospitilized patients were 0.01 % during the same period. The pathogens isolated in HACNSI were methicillin-resistant coagulase negative Staphylococcus 2, Acinetobacter spp. 2, Klebsiella spp. 1, Enterococcus faecium 1 and methicillin-sensitive Staphylococcus aureus 1. Pathogens were isolated from subdural empyema in one case, from blood in one case, from blood and cerebrospinal şuid in one case, from cerebrospinal şuid in 4 cases. Shunt revision were applied to 2 (13 %) patients and 3 (20 %) patients died. Though we need larger series to plan the initiation of ampiric antibiotic treatment, this study may be an important data for our hospital and country.
