Özet

Tüberküloz (TB) plörezi (TP), TB lenfadenitten sonra ikinci sıklıkta karşılaşılan akciğer dışı TB formu olup plevral efüzyonun en önemli etkenlerinden biridir. Bu çalışmada, TP şüpheli hastaların mikobakteriyoloji ve histopatoloji sonuçlarının demografik verileri ile değerlendirilmesi ve izole edilen mikobakteri suşlarının anti-TB ilaç direnç paternlerinin belirlenmesi amaçlanmıştır. Bu çalışmaya Kasım 2003-Ocak 2011 döneminde TP şüpheli 461 hasta arasından klinik materyalinde Ehrlich Ziehl-Neelsen boyama yöntemi ile aside rezistan bakteri (ARB) pozitifliği saptanan, kültüründe Mycobacterium tuberculosis kompleks (MTC) izole edilen ve histopatolojik olarak TP tanısı alan hastalar dahil edilmiştir. TP şüpheli 461 hastanın 69una (% 14.9) mikrobiyolojik ve histopatolojik incelemeler ile TP tanısı konulmuştur. Hastaların 41i (% 59.4) erkek, 28i (% 40.6) kadın ve yaş ortalamaları 44 (6-84) olarak bulunmuştur. TP tanılı 69 hastanın 35inde (% 50.7) kültür pozitifliği, altısında (% 8.6) ARB pozitifliği ve 52sinde (% 75.3) histopatolojik inceleme ile kronik granülomatöz inflamasyon saptanmıştır. Kültür pozitif 35 hastanın ikisinde (% 5.7) streptomisin (SM), birinde (% 2.9) izoniazid (İNH) ve birinde (% 2.9) etambutol, birinde (% 2.9) ise hem SM hem de İNH direnci belirlenmiştir. Hastaların hiçbirinde rifampisin direnci belirlenmemiştir. Geri kalan 30 (% 85.6) hastada ise primer anti-TB ilaçlara direnç tespit edilmemiştir. Özgeçmişine ve klinik bilgilerine ulaşılabilen 54 (% 78.3) hastada rastlanan en sık semptomlar sırasıyla nefes darlığı, öksürük ve göğüs ağrısı olarak belirlenmiştir. Hastaların yedisinde (% 13) önceden geçirilmiş TB, altısında (% 11.1) ise aile içinde ya da yakın çevresinde TB öyküsü saptanmıştır (yakın temas). En sık altta yatan hastalık diabetes mellitus olarak bulunmuştur. Plevral örneklerde TP tanısında kullanılan kültür, bakterinin üretilmesi ve ilaç duyarlılık paterninin belirlenmesi açısından güvenilir ve vazgeçilmez bir yöntemdir. Plevral örneklerin mikrobiyolojik ve histopatolojik yöntemlerle birlikte incelenmesi ile tanısal verimlilik ciddi oranda artmaktadır.

Abstract

Tuberculous pleurisy (TP) is the second common form of extrapulmonary tuberculosis (TB) after TB lymphadenitis and it is one of the major causes of pleural effusion. In this study, we aimed to evaluate the mycobacteriological and histopathological results and demographic data of patients with suspected TP and to determine the anti-TB drug susceptibility patterns of these mycobacterial strains. Between November 2003 and January 2011, of the 461 patients with suspected TP the ones who had acid fast bacilli (AFB) by Ehrlich Ziehl-Neelsen staining method in clinic material, Mycobacterium tuberculosis complex (MTC) in culture and histopathologically diagnosis as TP were included in this study. Of the 461 patients with suspected TP, 69 patients (14.9 %) were diagnosed as TP with microbiological and histopathological evaluations. Of these patients, 41 (59.4 %) were male, 28 (40.6 %) were female and the mean age was 44 (6-84) years. Of the 69 TP patients, 35 (50.7 %) patients were culture positive, six (8.6 %) patients were AFB positive and chronic granulomatosis inflammation was detected in 52 (75.3 %) patients with histopathological examination. In two (5.7 %) patients streptomycin (SM) resistance, in one (2.9 %) patient isoniazid (INH) resistance, in one (2.9 %) patient ethambutol resistance and in one (2.9 %) patient both SM and INH resistance was detected in culture positive 35 patients. Rifampicin resistance was not detected in the strains. In the remaining 30 (85.6 %) patients, primary anti-TB drug resistance was not detected. The most common symptoms among the 54 (78.3 %) patients,whose resume and clinical findings could be accessed, were respiratory disorder, cough and chest pain respectively. Seven (13 %) patients had previous history of TB, and six (11.1 %) of all, had domestic TB history (close contact). Diabetes mellitus was the most common underlying disease. Pleural sample culture that is used for diagnosis of TP, is a reliable and indispensable method in terms of growth of bacilli and determination of drug sensitivity patterns. Additionally, diagnostic efficiency increases significantly when the pleural samples are evaluated both with microbiological and histopathological methods.
