Akut miyokard infarktüsünde serum demir, demir bağlama kapasitesi ve ferritin düzeylerinin önemi

Özet
Yapılan çalışmalarda akut miyokard infarktüsünde (AMI) serbest demirin post-iskemik reperfüzyon hasarında önemli rolü olan serbest oksijen radikallerinin (SOR) oluşumunda önemli bir katalizör rolü olduğu ileri sürülmüş ve bu sav hayvan deneyleriyle desteklenmiştir. Demir düzeylerinin AMI'deki seyir ve rollerini araştırmak için yaptığımız bu çalışmada AMI' leriyle AMI dışı kardiyak hastalar arasındaki ve yaygın ile sınırlı infarktlardaki serum demir,serum demir bağlama kapasitesi (SDBK) ve ferritin düzeylerini karşılaştırdık. Çalışmamız hepsi koroner yoğun bakım ünitesine (KYBÜ) kabul edilen, 68 AMI'lu ve 17 AMI dışı kardiyak hastanın 0-24 saat, 3-5 gün ve 10.14. günler arasındaki demir, SDBK ve ferritin düzeylerini ölçerek, prospektif olarak yaptık. AMI'de serum demir, SDBK, düzeyleri başlangıca ve kontrol grubuna göre 3-5. günlerde anlamlı bir şekilde düşük bulundu, sonra yükselme eğilimine girmesine rağmen, 10.14. günlerde de anlamlı düşüklük devam etmiştir. Ferritin de AMI'nde 3-5 günlerde başlangıca ve kontrol grubuna göre anlamlı derecede yükselmiş, bu yükseklik 10-14. günlerde düşme eğilimine girmiş, ancak başlangıç değerine göre anlamlıya yakın yükseklik devam etmiştir. Geniş infarktlı grupta elde edilen ferritin değerlen sınırlı infarktlı gruba göre anlamlı olarak yüksek bulunmuştur. Bu bulgular literatürde adı geçen çalışmalarla uygunluk göstermektedir.
Abstract
In many studies, free iron is supposed to have a great role at the post-ischemie reperfusion damage in AMI, due to its effect as a catalyser for the production of the free oxygen radicals, as this theory is supported by animal studies. Our aim was to find out the role of serum level of iron in the clinical course of AMI. For this reason, we compared the iron, iron binding capacity (IBC) and ferritin levels of the cardiac patients with AMI, without AMI and the limited plus the widely infarcted ones. We measured the 0-24 hour, between 3 to 5 days and between 10 to 14 days iron, iron binding capacity and ferritin levels of the 85 cardiac patients; of which 68 AMI and 17 without AMI; who have been hospitalized at our coronary intensive care unit for a certain limited time. In the patients with AMI, serum iron, IBC levels were evidently low in the 3 to 5 day period when compared with the initial and the control group values. The low levels were still observed in the 10 to 14 day; although the values slowly have begun to rise. In the patient group with a history of recent AMI; ferritin values increased between the 3 to 5 days when compared with the initial and control group values. A tendency to decline becomes clear between the 10 to 14 days; but the values were still at a higher level than the starting ones. The ferritin values in the widely infarcted group were still quite higher than the group with the limited infarction. These results were perfectly matched with the previous studies.
