Özet

Amaç: Bu çalışmada santral obezite göstergesi olan bel çevresinin alt üriner sistem semptomları, erektil disfonksiyon ve metabolik sendrom komponentleri ile olan ilişkisini incelemeyi amaçladık. Gereç ve yöntem: Orta-şiddetli işeme şikayetleri ile başvurmuş, daha önceden Benign Prostat Hiperplazisi tanısı olmayan ve Prostat spesifik antijen değeri 102 cm) 3 gruba ayrıldı. Her gruptaki hastaların yaşı, ağırlığı, Uluslararası prostat semptom skoru değeri, prostat volümü, idrar akım hızları, Prostat spesifik antijen değeri ve lipid profili ayrı ayrı belirlendi. Ayrıca her gruptaki hastaların kronik sistemik hastalık ve erektil fonksiyon durumu hakkındaki kayıtlı bilgileri de incelendi ve bu veriler arasındaki ilişkiler araştırıldı. Bulgular: Hastaların yaş ortalaması 60,6±7,8 yıl idi. 17 (%32,69) hastada diyabet, 22 (%42,30) hastada hipertansiyon, 13(%25) hastada koroner arter hastalığı ve 24 (%46.15) hastada erektil disfonksiyon olduğu görüldü. Bel çevresi ile yaş, Uluslararası prostat semptom skoru, vücut ağırlığı, prostat volümü, trigliserid ve LDL kolesterol değerleri arasında pozitif korelasyon saptanırken; bel çevresi ve akım hızları arasında negatif korelasyon saptandı. Bel çevresine göre 3 gruba ayrılan hastaların verileri analiz edildiğinde bel çevresi arttığında alt üriner sistem semptomları ve seksüel fonksiyonların kötüleşmekte olduğu görüldü. Sonuç: Bel çevresi artışının işeme ve seksüel fonksiyonları olumsuz etkilediği ve diyabet, koroner arter hastalığıve hipertansiyon ve metabolik sendrom sıklığının arttığı saptandı.

Abstract

Objectives: In this study, we aimed to evaluate the relationship between waist circumference, which is as an indicator of central obesity, and lower urinary tract symptoms, erectile dysfunction and metabolic syndrome components. Materials and methods: The data of 52 patients with moderate-severe lower urinary tract symptoms, who had no previous diagnosis of benign prostatic hyperplasia and prostate specific antigen <4 ng/ml were retrospectively analyzed. The patients were divided in 3 subgroups according to the waist circumference (<94 cm, 94-102 cm, >102 cm). Age, weight, urine flow rates, prostate volume, and prostate specific antigen value and lipid profiles were separately determined for all patients. Data on chronic systemic diseases and erectile function was also evalu- ated and possible relations between them were investigated. Results: The mean age of patients was 60,6±7,8 years. It was determined that diabetes in 17 (%32,69), hypertension in 22 (%42,30), coronary artery disease in 13 (%25) and erectile dysfunction in 24 (%46.15) patients. Positive correlations were found between waist circumference and the parameters of age, International Prostate Symptom Score, body weight, prostate volume, triglyceride and LDL-cholesterol levels. Nevertheless, there was a negative correlation between waist circumference and urine flow rates. While the data of all subgroups were compared, it was seen that increased waist circumference was with worsened lower urinary tract symptoms and sexual function. Conclusions: Increased waist circumference had negative impacts on lower urinary tract symptoms and sexual functions, and it was with increasing frequency of diabetes, coronary heart disease, hypertension and metabolic syndrome.
