Tüberkülozlu 44 çocuk hastanın epidemiyolojik, klinik ve prognoz yönünden değerlendirilmesi

Özet

Amaç: Ocak 2003 - Eylül 2006 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Hastanesi, Dr. Behçet Uz Çocuk Hastanesi ve Tepecik Eğitim ve Araştırma Hastanesi’nin Çocuk Enfeksiyon Bölümlerinde tüberküloz hastalığı tanısı alarak tedavi başlanan hastaların epidemiyolojik özellikleri, başvuru semptomları, fizik muayene bulguları, tanıları, laboratuar bulguları, tanı metotları, tedavi rejimleri ve prognozlarını karşılaştırmak. Gereç ve Yöntem: Tüberküloz tanısı alan 44 çocuk hastanın kayıtları retrospektif olarak değerlendirildi. Bulgular: Bu dönemde tedavi alan 44 hastanın 27’si pulmoner tüberküloz, 17’si ekstrapulmoner tüberküloz olgularıydı. Pulmoner tüberküloz olgularının 23’ü primer pulmoner tüberküloz iken, ekstrapulmoner tüberkülozu olgularında en sık merkezi sinir sistemi tüberkülozu saptandı. Hastaların kliniğe en sık başvuru şikayetleri ateş, iştahsızlık - kilo kaybı ve öksürük idi. Hastalardan 8’inde (%18.1) bakteriyolojik incelemelerle basil gösterildi. Sonuç: Hastaların başvuru şikayetlerinde ve laboratuar bulgularında literatür ile uyumlu sonuçlar elde edilirken, çalışmamızda kültür pozitifliği oranı daha yüksek olarak saptanmıştır. Klinik, radyolojik ve histopatolojik bulgular ile tüberküloz tanısının kuvvetle muhtemel desteklenen olguların çalışmaya alınmış olması nedeniyle kültür pozitiflik oranı daha yüksek bulunduğu düşünülmektedir.

Abstract

Aim: To evaluate the epidemiological findings, submission symptoms, physical examination findings, laboratory findings, diagnostic methods, diagnosis, treatment regimens and prognosis of the patients with tuberculosis which were recruited from the pediatrics department of three major hospitals in Izmir ( Ege University Faculty of Medicine, Dr Behcet Uz Children Hospital Tepecik Social Workers Hospital) between the period of January 2003 and September 2006. Materials and Methods: Fortyfour children with tuberculosis were included in the study and evaluated retrospectively. Results: Twentyseven of the 44 patients were pulmonary tuberculosis patients and 17 patients were diagnosed as having extrapulmonary tuberculosis. Twentythree of the 27 pulmonary tuberculosis patients were primary pulmonary tuberculosis and the most frequent extrapulmonary tuberculosis type was central nervous system tuberculosis. Fever, loss of weight and coughing were the patients’ major complaints. Mycobacterium tuberculosis was detected in 8 patients (8,1%) on bacteriological analysis. Conclusion: In our study the distribution of complaints and laboratory findings were consistent with the literature, whereas the M. tuberculosis detection rate in the culture was higher than reported in the literature. This might be due to selection of the patients whose diagnoses were strongly supported by the clinical, radiological and histopathological findings.
