Postpartum maternal depresyon nedenleri ve preterm bebeklerde büyüme üzerine etkileri

Özet

Amaç: Postpartum maternal depresyona neden olabilecek değişik risk faktörlerini ve maternal depresyonun preterm bebeklerin büyümeleri üzerine olan etkisini araştırmak. Yöntem: Kırkiki preterm yenidoğan ve annesi prospektif olarak çalışmaya alındı. Annelere bebekleri taburcu olmadan önce "BECK DEPRESYON ENVANTERİ" uygulandı. Skorlamada >17 alanlar depresyonu olan grubu oluşturdu. Depresyonu olan anneler olmayanlarla gestasyon yaşı, doğum ağırlığı, hastanede kalma süresi, yoğun bakımda kalma süresi, beslenme şekli, tam enteral beslenmeye geçiş zamanı gibi bebeğe ait ve anne yaşı, eğitimi, ekonomik durumu, gebeliğin gerçekleşme şekli, çoğul gebelik, medeni durum gibi bazı maternal ailesel risk faktörleri açısından karşılaştırıldılar. Bebeklerin büyümeleri, hastaneden taburcu olduktan sonraki düzeltilmiş yaşlarına göre 1., 2., 3. ve 4. aylardaki kilo alma hızları, baş çevresi ve boy artış miktarları ile değerlendirildi. Sonuçlar: Yirmi üç annenin (% 54.7) depresif bulguları olduğu saptandı. Preterm doğum dışında, anneye ve bebeğe ait faktörlerin hiçbirinin maternal depresyondan sorumlu olmadığı bulundu (p>0.05). İzlemde annelerinde depresyon olan bebeklerin kilo alma hızlarının hastaneden taburcu olduktan 1 ay sonra diğer gruba göre düşük olduğu (p 0.05). Sonuç: Çalışmamız annelerinde depresyon olan preterm bebeklerin özellikle hastaneden taburcu olduktan sonraki ilk 1 ay içerisinde yetersiz ağırlık artışı açısından risk altında olduğunu göstermiştir. Bu sonuçlar, maternal depresyonun istenmeyen etkilerinin gelişmemesi için bir yandan bebeklerin büyümelerinin yakından izlenmesi diğer yandan da annelerine psikiyatrik destek verilmesi gerekliliğini düşündürmüştür.

Abstract

Aim: To investigate various factors that might be responsible for postpartum maternal depression and also the effect of maternal depression on growth velocity of preterm infants. Methods: Fortytwo preterm infants and their mothers were prospectively enrolled in the study. Mothers were assessed with "BECK DEPRESSION INVENTORY" at the time of discharge. A score of >17 was accepted as maternal depression. Depressive mothers were compared with non depressive ones on the basis of infant (gestational age, birth weight, duration of hospitalization, duration of neonatal intensive unit care, type of feeding, initiation of full enteral feedings) and maternal-familial factors (maternal age, parent's education, economic status, parity, multiple gestation, marriage status). Growth velocity, on the basis of weight gain, increase in head circumference and height were compared between the two groups at 1, 2, 3 and 4 months of corrected age. Results: Twentythree mothers (54.7 %) were diagnosed as having depression. None of the infant and maternal factors, other than preterm delivery, were found responsible for maternal depression (p>0.05). On follow-up of growth velocity, the rate of weight gain was found lower in infants of depressive mothers in the first month after discharge (p<0.05). However no significant differences were detected between growth rates of two groups at 2, 3 and 4 months of corrected age. Conclusion: We observed that preterm infants with maternal depression had greater risk of inadequate weight gain especially during the first month after discharge. So, follow-up programs should incorporate support services for depressive mothers of these infants and careful infant nutrition monitorization should be done to minimize undesirable effect of maternal depression.
