The effects of iron deficiency anemia on cognitive function in children

Özet

Amaç: Demir eksikliği anemisinin (DEA) çocuklarda üst bilinçsel, kognitif fonksiyonlar ile IQ oranı üzerine olan etkilerinin araştırılması amaçlandı. Yöntem ve Gereçler: İstanbul Üniversitesi İstanbul Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Genel Polikliniği'ne başvuran demir eksikliği anemisi tespit edilmiş 30 vaka çalışmaya alındı. Vakaların yaşları 6-12 yaş arasındaydı. Doğum hikayesinde özellik saptanmayan, nöromotor gelişimleri normal, kronik hastalık hikayesi olmayan, daha önce demir tedavisi almamış vakalardı. Demir eksikliği anemisi olmayan 30 vaka da kontrol grubunu oluşturdu. DEA grubumuza WISC-R zeka testi yaptıktan sonra, 5 mg/kg/gün dozunda ferrik (III) hidroksipolimaltoz içeren demir preparatı ve 45 mg/gün C vitamini içeren multivitamin verildi. DEA düzelen vakalara ilk testten 6 ay sonra IQ testinin tekrarı yapıldı. Anemi açısından kan değerleri normal olan kontrol grubu vakalarımıza WISC-R zeka testi yapıldı. DEA grubunun tedavi öncesi ve sonrası IQ sonuçları ve DEA grubunun kontrol grubuna kıyasla IQ sonuçları değerlendirildi. Bulgular: DEA grubunun yaş ortalaması 9.1±1.9 yıl, kontrol grubunun yaş ortalaması 8.9±1.5 yıldı. DEA grubunun % 63'ü erkek, % 37'si kız; kontrol grubunun % 60'ı erkek, % 40'ı kız çocuğuydu. İki grup arasında yaş ve cinsiyet açısından anlamlı fark bulunmadı (p>0.05). DEA grubunun tedavi öncesi ortalama Hb: 10.2±1.4 g/dL, Hct: % 31.6±3.4, MCV: 68.5±7.2fL, Demir: 38.6±9.9 mg/dL, TDBK: 414.9±53.5, Ferritin: 6.7±3.0 ng/mL'ydi. 4-6 aylık tedavi sonrasında Hb: 12.3±0.8 g/dL, Hct: % 37.6±2.4, MCV: 78.3±6.6fL, Demir: 64.8±19.8 mg/dL, TDBK: 335.8±64.1, Ferritin: 29.3±20.4 ng/ml bulundu. Kontrol grubunun ortalama Hb: 12.7±0.5 g/dL, Hct: % 37.7±1.9, MCV: 78.4±2.7fL, bulundu. DEA vakalarımızın total IQ sonucu kontrol grubundan 12.9 puan daha düşüktü ve iki grup arasında istatistiksel anlamlı fark vardı (p 0.05). Demir eksikliğinin dikkat, hafıza, öğrenmeyi özellikle etkilediği görüldü. DEA grubu ve kontrol grubu arasında anne eğitimi açısından istatistiksel anlamlı fark elde edildi (p 0.05). Sonuç: DEA çocuklarda üst bilinçsel fonksiyonları etkileyerek IQ oranının düşmesine neden olur. Demir tedavisi ile bu oran yükselmekle birlikte kalıcı olarak kontrol grubuna göre daha düşüktür. Bu fark istatistiksel anlam taşımasa da DEA öncelikle önlenmesi gereken bir toplum sağlığı sorunudur.

Abstract

Objective: We aimed to examine the effects of iron deficiency anemia (IDA) on cognitive functions and IQ in children. Methods: Thirty cases between 6 to 12 years old, referring to outpatient clinic of Istanbul University, Istanbul Faculty of Medicine, Department of Pediatrics with diagnosis of iron deficiency anemia. IDA cases whoes have not been previously treated, without any particularity in birth history and chronic disease, whoes have normal neuromotor development, were included to the study. Control group consisted of 30 cases without anemia. In IDA group, following WISC-R IQ test, iron preparation containing ferric (III) hydroxypolymaltose at a dose of 5 mg/kg/day and multivitamin containing vitamin C at a dose of 45 mg/day were given. WISC-R mental test has been done in control cases having normal blood values in respect of anemia. Pre- and post-treatment IQ scores of IDA group were evaluated and IQ scores of IDA group were compared to control group. Results: Mean age of IDA group was 9.1±1.9 years, and that of control group was 8.8±1.5 years. 63 % of IDA group were boys and 37 % were girls; 60 % of the control group were boys and 40 % were girls. There was no significant difference between two groups in respect of age and sex. Pre-treatment mean hemogram values of IDA group were; Hb: 10.2±1.4 g/dL, Hct: % 31.6±3.4, MCV: 68.5±7.2fL, serum iron: 38.6±9.9 mg/dL, serum total iron binding capacity (TIBC): 414.9±53.5, Ferritin: 6.7±3.0 ng/mL. Following 4 to 6 months of treatment post-treatment mean values were; Hb: 12.3±0.87 g/dL, Hct: % 37.6±2.4, MCV: 78.3±6.6fL, TDBK: 335.8±64.2, Ferritin: 29.3±20.4 ng/mL. Mean hemogram values of control group were; Hb: 12.7±0.6 g/dL, Hct: % 37.7±1.9, MCV: 78.4±2.8fL. Total IQ score of IDA group was 12.9 points lower than that of the control group and this was statistically significant between groups (p<0.01). Although we found a highly significant increase of 4.8 points in pre- and post-treatment IQ in IDA group (p<0.001), post treatment IQ score of the IDA group was 8.1 points lower than that of the control group. This difference of 8.1 points was not statistically significant (p>0.05). There was a significant difference between pre-treatment IDA group and control group in subunits of IQ test such as general knowledge, judgment, similarity, vocabulary, picture completing and coding. A significant improvement was found especially in these subtests following treatment. Conclusion: Iron deficiency anemia in children is much more than a simple decrease in hemoglobin level; it can affect long-term cognitive function. The WISC-R intelligence test subsets and pre- and post-treatment IQ scores of the IDA group were significantly differing from control group.
