Özet

Amaç: Perkütan böbrek biyopsisi, erişkinlerde olduğu gibi çocuklarda da glomerül hastalıkları başta olmak üzere birçok nefropatinin tanısında, prognozun belirlenmesinde ve tedavi seçiminde yol göstericidir. Yöntem: Bu çalışmada, hastanemiz çocuk nefroloji kliniğinde Mayıs 1996-Şubat 2004 tarihleri arasında yapılan 161 perkütan böbrek biyopsisi retrospektif olarak değerlendirildi. Bulgular: Vakalarımızın yaş ortalaması 8.7±4.0 (0.5-17) yıl olup, 81'i erkek, 80'i kızdı. Biyopsi endikasyonları; 74 vakada (% 46) proteinüri, 35 vakada (% 22) hematüri, 20 vakada (% 12) hematüri ve proteinüri, 20 vakada (% 12) vaskülit ve 12 vakada (% 8) kronik böbrek yetmezliği etiyolojisini belirlemekti. Biyopsi öncesi tüm vakalarda, ultrasonografi (USG) ile her iki böbreğin varlığı ve normal yerleşimli olup olmadığı saptandı. Biyopsiler USG eşliğinde biyopsi probu ile 14 G Bard biyopsi iğneleri ile yapıldı. Örnekler ışık, elektron ve immunfloresan mikroskobunda incelendi. Biyopsi materyalleri ortalama 21.6±8.9 (6-67) glomerül içeriyordu ve patolojik inceleme için yeterliydi. Hastalar biyopsi sonrası 48 saat hastanede yatırılarak komplikasyon gelişimi açısından izlendi. Yaşamı tehdit eden komplikasyona rastlanmadı. Makroskopik hematüri % 14.9, perirenal hematom ise % 9.3 oranında gözlendi. Patolojik inceleme sonucunda vakaların 29'unda fokal segmental glomerüloskleroz, 25'inde mezangioproliferatif glomerülonefrit, 20'sinde Henoch-Schönlein nefriti, 14'ünde membranoproliferatif glomerülonefrit, 11'inde kresentik glomerülonefrit, 9'unda IgA nefriti, 8'inde minimal değişiklik hastalığı, 7'sinde kronik glomerülonefrit, 5'inde Alport sendromu, 5'inde lupus nefriti, 3'ünde amiloidoz, 8'inde diğer nedenler ve 17'sinde normale yakın görünüm saptandı. Sonuç: USG eşliğinde yapılan perkütan böbrek biyopsisinin çocuklarda da güvenilir ve tanı koydurucu olduğu sonucuna varıldı.

Abstract

Aim: Percutaneous renal biopsy has become a diagnostic tool in the assessment of nephropathies, especially glomerular diseases and a cornerstone for evaluating the prognosis and deciding on treatment, in children as well as in adults. Methods: In this study, 161 percutaneous renal biopsies performed at the Pediatric Nephrology Unit of our hospital between May 1996 and February 2004 were retrospectively evaluated. Results: The mean age of the patients was 8.7±4.0 (0.5-17) years (81 boys, 80 girls). The main indications for renal biopsies were isolated proteinuria in 74 patients (46 %), hematuria in 35 (22 %), hematuria and proteinuria in 20 (12 %), vasculitis in 20 (12 %) and chronic renal failure in 12 (8 %). Prior to the biopsy procedure, all patients underwent an ultrasonographic examination to assess if both kidneys are available in the normal anatomical position. The biopsy procedure was performed with ultrasound guidance using the automated Bard Biopty instrument (14-gauge needles). All the specimens were examined by light, immunofluorescence and electron microscopies. The mean number of glomeruli per each biopsy specimen were calculated to be as 21.6±8.9 (6-67) and were sufficient for examination. The patients remained in the hospital for about 48 hours after the procedure. No life threatening complications were attributable to the biopsy procedure. Macroscopic hematuria (14.9 %) and perirenal hematoma (9.3 %) constituted the most frequent complications. According to the pathological examination of the specimens the diagnostic distribution were as follows: focal segmental glomerulosclerosis (n=29), mesangioproliferatif glomerulonephritis (n=25), Henoch-Schonlein nephritis (n=20), membranoproliferatif glomerulonephritis (n=14), cresentic glomerulonephritis (n=11), IgA nephritis (n=9), minimal change disease (n=8), chronic glomerulonephritis (n=7), Alport syndrome (n=5), Lupus nephritis (n=5), amiloidosis (n=3), other disorders (n=8), normal (n=17). Conclusion: Percutaneous renal biopsy using the automated biopsy device guided with ultrasonography is a safe diagnostic procedure in children.
