Brusellozun serolojik tanısında immunocapture aglütinasyon testinin değerlendirilmesi

Özet
Amaç: Brusellozun tanısında kültür yöntemleri önemli olmakla birlikte, geç sonuç alınması, antibiyotik kullanımı ile duyarlılığın düşük olması, kültür olanaklarının her zaman bulunmaması nedenleriyle, tam pratik olarak serolojik yöntemlerle konulmaktadır. Bu çalışma brusellozun serolojik tamsmda standart tüp aglütinasyon (STA), Coombs anti- Brucella (Coombs) ve immunocapture aglütinasyon (Brucellacapt) yöntemlerinin birbirleriyle uyumu ve korelasyonlarını değerlendirmek amacıyla yapılmıştır. Materyal ve Metod: Çalışmaya, bruselloz öntamlı ve Rose-Bengal (RB) test sonucu pozitif bulunmuş 130 serum örneği ile kontrol grubu olarak daha önceden Brucella ile karşılaşmamış veya bruselloz öyküsü vermeyen sağlıklı 80 kişiden alman serum örnekleri dahil edildi. STA, Coombs ve Brucellacapt yöntemlerinin birbirleriyle uyumları kappa değerleri ve Pearson korelasyon katsayıları yardımıyla değerlendirildi. Bulgular: Her üç yöntemin titrasyon değerleri arasmda "çok kuvvetli" korelasyon bulundu (STA-Coombs; r=0.84,pO.001, STA-Brucellacapt; r=0.76, pO.001, Coombs-Brucellacapt; r=0.84, pO.001). Söz konusu yöntemlerin Brucella tanısı koymadaki uyumları ise "mükemmel" ve "iyi" arasında değişmekteydi (STA-Coombs; kappa değeri:0.77, p<0.001, Coombs-Brucellacapt; kappa değeri:0.67, p<0.001, STA-Brucellacapt; kappa değeri:0.48, pO.001). Sonuç: Literatürdeki diğer çalışmalar ve bu çalışmanın sonuçlan; Brucellacapt'in, Çoombs testine benzer bir şekilde, alternatif bir test olarak brusellozun hem tanısında hem de izlenmesinde güvenle kullanılabileceğini düşündürmektedir.
Abstract
Objective: Although culture is an important method in diagnosis of brucellosis, it is time consuming, can not be easily available and may be less sensitive when antibiotics are involved. Thus, serological methods are preferred for the diagnosis of brucellosis. The aim of this study was to assess the correlations and levels of agreement between standard tube agglutination (STA), Coombs anti-Brucella (Coombs) and immunocapture agglutination methods in the serological diagnosis of brucellosis. Materials and Methods: One hundred thirty serum samples of patients with clinical features suggestive of brucellosis who had positive Rose-Bengal (RB) test; and eighty serum samples from healthy donors with no history of brucellosis or exposure to brucella were included in the study. The correlations and levels of agreement between STA, Coombs and Brucellacapt methods were evaluated by Pearson correlation coefficients and kappa values. Results:There were "very strong" correlations between the titration values of the three methods (STA-Coombs; r=0.84, pO.001, STA-Brucellacapt; r=0.76, p<0.001, Coombs-Brucellacapt; r=0.84, p<0.001). The levels of agreement between three methods varied between "excellent" and "good" (STA-Coombs; kappa:0.77, p<0.001, Coombs-Brucellacapt; kappa:0.67, p<0.001, STA-Brucellacapt; kappa:0.48, p<0.001).
