Özet

Amaç: Atriyal fibrilasyon, koroner baypas ameliyatlarından sonra sık karşılaşılan bir durumdur. Bu çalışmanın amacı koroner arter has‐talığı nedeniyle ameliyat edilen hastalara farklı koroner baypas cerrahi teknikleri uygulanarak, postoperatif dönemde gelişen atriyal fib‐rilasyon sıklığının ve nedenlerinin incelenmesidir.Gereç ve Yöntem: Çalışma dahilinde 51 hastaya pompa destekli atan kalpte koroner baypas cerrahisi ve 51 hastaya konvansiyonel yön‐temle koroner baypas cerrahisi uygulanmış, toplamda 102 hastaya ait veriler retrospektif olarak karşılaştırılmıştır. Cerrahi teknik yö‐nünden farklılık gösteren 2 gruptaki hastalar postoperatif dönemde atriyal fibrilasyon gelişen ve gelişmeyen hastalar olmak üzere 2 gru‐ba ayrılarak atriyal fibrilasyon gelişimine etki edebilecek parametreler karşılaştırılmıştır.Bulgular: Pompa destekli grupta 3 hastada (%5,88), koroner arter baypas greftleme grubunda ise 15 hastada (%29,41) postoperatif at‐riyal fibrilasyon saptanmıştır (p=0,002). Çalışmamızda atriyal fibrilasyon gelişimi açısından pompa kullanımı ve süresinin, aorta kros‐klemp konulmasının ve vücut ısısının düşürülmesinin etkili olduğu sonucuna varılmıştır.Tartışma: Nativ koroner kan akımının sağlandığı ve kalbin çalışmasına müsade edilen pompa destekli atan kalpte cerrahi baypas tekni‐ğinin atriyal fibrilasyon gelişim riski açısından konvansiyonel teknikle yapılan koroner baypas ameliyatlarına göre daha koruyucu olabi‐leceği sonucuna varılmıştır. Benzer çalışmalar olmakla birlikte kesin yorum yapılabilmesi için daha büyük hasta serilerinin olduğu pros‐pektif çalışmaların yapılmasının gerekliliği düşünülmüştür.

Abstract

Objective: Atrial fibrillation is a frequent situation seen after coronary bypass surgery. The aim of this study analyzing the frequency and causes of postoperative atriyal fibrillation by using different technics of coronary bypass surgery becaouse of coronary arterial dis‐ ease. Material and Method: We analyzed the datas of 102 patients retrospectively who underwent coronary artery bypass surgery, includ‐ ing 51 on pump/beating heart and 51 convensional technique. The patients who underwent two different surgical technic were divided into two groups according to postoperative atrial fibrillation development or not for comparing different parameters which can cause atrial fibrillation. Results: Postoperative atrial fibrillation detected 3 patient (5,88%) in the on pump/beating heart group, 15 patient (29,41%) on the con‐ vensional technique group (p=0,002). In our study we found that using and duration of cardiopulmonary bypass, cross‐clamping of aorta and lowering the body temperature are statistically significant variables that has impact on postoperative atrial fibrillation. Conclusion: As a result of these data, we believe that the on pump/beating heart coronary bypass surgery which allows the blood flow through the native coronary vessel and functioning of heart is much safer than the conventional coronary surgery. Although there are similar studies to our study prospective studies of large series must be performed to make definite comments.
