Optik Nörit Olan ve Olmayan Multipl Skleroz Hastalarında Retina Sinir Lifi, Ganglion Hücre Tabakaları ile Fovea Kalınlıklarının Optik Koherans Tomografi ile Değerlendirilmesi

Özet

Amaç: Optik nörit hikayesi olan ve olmayan multipl skleroz hastalarında, retina sinir lifi, ganglion hücre tabakası ve fovea kalınlıklarını değerlendirmek.Gereç ve Yöntem: Kırk iki optik nörit hikayesi olan (MSON+) ve 22 optik nörit hikayesi olmayan (MSON-) toplam 64 multipl sklerozhastası ile 12 sağlıklı kontrol çalışmaya dahil edildi. Optik koherans tomografi ile retina sinir lifi, ganglion hücre tabakası, fovea kalınlıkları ölçüldü. P<0,05 altındaki değerler istatistiksel olarak anlamlı kabul edildi.Bulgular: MSON+ grubunda 84, MSON- grubunda 44 ve kontrol grubunda 24 göz mevcuttu. Grupların yaş ortalamaları arasında istatistiksel anlamlı fark yoktu. Retina sinir lifi kalınlıkları, MSON+ grubunda ve MSON- grubunda kontrol grubuna göre anlamlı derecededüşüktü (p<0.01). Ganglion hücre tabakası kalınlığı MSON+ grubunda 86,2±12,4 μ, MSON- grubunda 94,4±8,7 μ ve kontrol grubunda99,2±4,6 μ ölçüldü. Gruplar arasında yaş ortalamaları benzer olmasına rağmen MSON+ grubu ganglion hücre tabakası kalınlıkları,MSON- ve kontrol grubuna göre istatistiksel olarak anlamlı düşük bulundu (p<0,05 her bir grup için). MSON- grubu ile kontrol grubuganglion hücre tabakası kalınlıkları arasında, istatistiksel anlamlı fark bulunmadı. ON geçiren hastalarda foveal, perifoveal ve parafoveal kalınlıklar sağlıklı kontrollere göre (sırası ile p=0,001, p=0,001, p=0,027) ve foveal ve perifoveal kalınlıklar ON geçirmeyen hastalara göre anlamlı olarak düşük bulundu (sırası ile p=0.002, p=0,003). Ancak MSON- hasta grubu ve MSON+ hasta grubu parafoveakalınlıkları arasında istatiksel olarak anlamlı fark bulunmadı (p=0,143).Sonuç: Retina sinir lifi ve fovea kalınlıkları, MSON- ve MSON+ grup hastalarında sağlıklı kontrollere göre düşük bulundu. Ganglionhücre tabakası kalınlıkları MSON+ grubunda, MSON- grubuna ve sağlıklı kontrollere göre istatistiksel olarak anlamlı düzeyde düşükbulundu.

Abstract

Objective: To analyse the thickness of the retinal nerve fiber layer, ganglion cell layer and fovea in patients with multiple sclerosis, with and without a history of optic neuritis and healthy individuals. Material and Method: Sixty-four multiple sclerosis patients, 42 with a history of optic neuritis (MSON+) and 22 without a history of optic neuritis (MSON-), and 12 age- and sex-matched healthy control subjects were included in the study. All subjects underwent ocular and retinal examination. The retinal nerve fiber layer, ganglion cell complex ganglion cell layer and foveal thicknesses were measured by optical coherence tomography. Between groups comparison was performed with p<0.05 indicating statistical significance. Results: There were 84 eyes in the MSON+ group, 44 eyes in the MSON- group and 24 eyes in the control group. The age of the patients did not statistically differ in between groups. The retinal nerve fiber layer in MSON+ and MSON- patient group was significantly thinner than the control group (p<0.05). The ganglion cell layer thickness was 86.2±12.4 μ in the MSON+ group, 94.4±8.7 in MSON- group and 99.2±4.6 μ in the control group. Despite controlling for age, the ganglion cell layer was significantly thinner in eyes with MSON+ compared to MSON- eyes and healthy controls (p<0.05 for each). No statistically significant differences were found between MSONgroup and control group with respect to ganglion cell layer. Patients with optic neuritis had a significantly lower foveal , perifoveal and parafoveal thicknesses (p=0.001, p=0.001, p=0.027 respectively) than healthy subjects and lower foveal and perifoveal thicknesess than patients without optic neuritis (p=0.002, p=0.003 respectively). However, there was no significant difference between the MSON-patient and MSON+ patient groups in the parafoveal thicknesses (p=0.143)
