Özet

Amaç: Alt göz kapağı tam kat defektlerinde modifiye Hughes tarsokonjonktival flep ile farklı flep ve greft tekniklerinin kombinasyonunsonuçlarını değerlendirmek.Gereç ve Yöntem: Ocak 2015 ile Ocak 2020 tarihleri arasında alt kapak bazal hücreli karsinom nedeniyle tam kat eksizyon uygulanıpmodifiye Hughes cerrahisi ile yeniden yapılandırılma uygulanan hastalar geriye dönük olarak değerlendirildi. Hughes flebin tek başınayetersiz kaldığı geniş defektlerde ek olarak kantotomi, kantoliz, ilerletme ve periost flepleri kullanılarak arka lamel rekonstrüksiyonu yapıldı. Ön lamel ise cilt-kas ilerletme flepleri veya cilt greftleri ile oluşturuldu. Ameliyat sonrası üst ve alt kapak pozisyonu, kontürü vekomplikasyonlar analiz edildi.Bulgular: Hastaların 3’ü kadın (%37,5), 5’i erkek (%62,5) idi. Hastaların ameliyat sırasındaki yaş ortalamaları 69,8±7,8 yıl (58-81 yıl),ameliyat sonrası ortalama takip süresi ise 13,5±5,0 ay (8-23 ay) idi. Arka lamel yeniden yapılandırılması için 4 hastada Hughes flep(%50) tek başına yeterli olur iken, dört hastada Hughes flebe ilave olarak kantotomi, kantoliz, ilerletme flepleri de (%50) kullanıldı.Ayrıca 2 hastada periost flepleri ile de (%25) kombinasyon uygulandı. Ön lamel yeniden yapılandırmasında ise 4 hastada cilt-kas ilerletme flebi (%50) yeterli olurken, diğer 4 hastada ise cilt grefti (%50) kullanıldı. Hiçbir hastada flep ve/veya greft ile ilgili bir komplikasyon saptanmadı. Tüm olgularda üst kapak pozisyonu ve kontüründe belirgin değişiklik izlenmedi. Alt kapak pozisyonu ve kontüründeise yalnızca bir hastada (%12,5) alt kapak retraksiyonu izlendi ancak lagoftalmi saptanmadı.Sonuç: Alt göz kapağı defektlerinde modifiye Hughes tarsokonjonktival flep ile farklı flep ve greft tekniklerinin kombinasyonu hem fonksiyonel hem de kozmetik açıdan etkili ve güvenli bir yöntem olarak kullanılabilir

Abstract

Objective: To evaluate the outcomes of modified Hughes tarsoconjunctival flap in combination with various flap and graft techniques for treatment of full-thickness lower eyelid defects. Material and Method: Patients who underwent modified Hughes procedure for reconstruction of full-thickness lower eyelid defects following excision of basal cell carcinoma between January 2015 and January 2020 were evaluated retrospectively. Canthotomy, cantholysis, advancement and periosteal flaps were used if Hughes flap was insufficient for reconstruction of posterior lamella. Skin grafts or skin-muscle advancement flaps were used for reconstruction of anterior lamella. Postoperative upper and lower eyelid position, contour and complications were analyzed. Results: The patients comprised of 3 women (37.5%) and 5 men (62.5%). Their mean age was 69.8±7.8 years (58-81 years), and the postoperative follow-up time was 13.5±5.0 months (8-23 months). Hughes flap was sufficient in 4 patients (50%) for reconstruction of posterior lamella. Canthotomy, cantholysis and advancement flaps were combined with Hughes flap in 4 patients (50%). Periosteal flaps were added in 2 patients (25%). While skin-muscle advancement flaps were sufficient in 4 patients (50%) for reconstruction of anterior lamella, skin grafts were used in 4 patients (50%). Any flap and/or graft related complications were not observed. The obvious changes of upper eyelid position and contour were not found postoperatively. Lower eyelid retraction was observed only one patient (12.5%), however lagophthalmos was not seen. Conclusions: Combination of modified Hughes tarsoconjunctival flap with various flap and graft techniques can be used as a functional and cosmetically effective and safe procedure in reconstruction of full-thickness lower eyelid defects.
