Keratokonus Hastalarında Yaş ve Klinik Evrenin Hızlandırılmış Korneal Çapraz Bağlama Tedavisi Etkinliği Üzerine Etkileri

Özet

Amaç: Progresif keratokonus tanısıyla hızlandırılmış yöntemle korneal çapraz bağlama tedavisi uygulanmış ve en az 12 ay takipli olanhastalar üzerinde korneal çapraz bağlama tedavisinin etkinliğinin yaş ve klinik evreye göre karşılaştırılması.Gereç ve Yöntem: Bu retrospektif çalışmaya 12-55 yaşları arasındaki, hızlandırılmış yöntemle korneal çapraz bağlama tedavisi uygulanan ve en az 12 ay takip edilmiş 54 hastanın 54 gözü dahil edilmiştir. Hastalar yaş ve keratokonus evresine göre 3 gruba ayrılmıştır. Kdeğerlerindeki değişim, yaş grupları ve keratokonus evresine göre karşılaştırılmıştır.Bulgular: Bu retrospektif çalışmaya hızlandırılmış yöntemle korneal çapraz bağlama tedavisi yapılan, yaş ortalaması 24,7 ± 9,84 (12-55)ve korneal çapraz bağlama sonrası ortalama takip süresi 13,0±2,63 (12-21) ay olan 54 hastanın toplam 54 gözü dahil edilmiştir.Operasyon öncesi ve sonrası ortalama K1 değerleri sırasıyla; 46,55±3,04 (42,0-57,11) D - 45,36 ± 3,31 (40,10-53,45) D (p 0,05). Keratokonus evrelerine göre gruplar arasında fark yoktu.Sonuç: Hızlandırılmış korneal çapraz bağlama tedavisinin etkinliği 18-31 yaş grubunda en fazladır. Klinik evreye göre etkinlikte bir farkbulunmamaktadır.

Abstract

Objective: The aim of this study was to compare the efficacy of accelerated corneal cross-linking treatment according to age and clinical stage in patients with progressive keratoconus. Material and Method: This retrospective study included 54 eyes of 54 patients were between the ages of 12-55 who had undergone accelerated cross-linking, and had a follow-up of at least 12 months. The patients divided into 3 groups according to age and keratoconus stage. The difference of K values were evaluated in terms of age and keratoconus stage. Results: A total 54 eyes of 54 patients who were treated with the accelerated corneal cross-linking were included. The mean age of all patients was 24.7±9.84 (12-55), and the follow-up period after the accelerated corneal cross-linking was 13.0±2.63 (12-21) months. Average of K1, K2, Kmean, Kmax, and topographic astigmatism values before and after the accelerated corneal cross-linking were; 46.55±3.04 (42.0-57.11) D-45.36±3.31 (40.10-53.45) D (p<0.001), 50.79±3.64 (45.10-61.36) D-49.31±4.10 (40.70-57.83) D (p<0.001), 48.63±3.20 (44.0-59.23) D - 47.24±3.56 (40.40-55.0) D (p<0.001), 55.92±4.34 (48.4-65.9) D-55.06±5.0 (44.40-70.80) D (p=0.006), and 4.23±1.93 (1.0-9.50) D-3.94±2.09 (0.20-10.20) D idi (p=0.097), respectively. In terms of age groups; K1, K2, and Kmean in the group 2 decerased higher than those in the other groups (p<0.05). According to the keratoconus stages, there was no difference between the groups. Conclusion: The effectiveness of accelerated corneal cross-linking treatment is highest in the 18-31 age group. There is no difference in effectiveness according to the clinical stage.
