Metastatik merkel hücreli karsinom

Özet

Merkel hücreli karsinom derinin primer nöroendokrin tü- mörüdür. Genellikle yaşlı hastalarda ve baş-boyun bölge- sinde görülmektedir. Seyrek tanı alan bu kanser; yüksek oranda lokal ve bölgesel yineleme ve sık uzak metastazlar ile seyreder. Bu çalışmada cerrahi eksizyon sonrası kalıntı tümör nedeniyle postoperatif radyoterapi ile tedavi edilen fakat 8 ay sonra bölgesel yineleme ve uzak metastazlarla seyreden progrese merkel hücreli karsinom olgusu bildiril- miştir. Olgumuzda olduğu gibi sık lokal, bölgesel yineleme ve uzak metastaz yapan merkel hücreli karsinomun tedavisinde rad- yoterapi ve kemoterapinin yeri, literatür bilgileri dâhilinde tartışılmıştır.

Abstract

Merkel cell carcinoma is the primary neuroendocrine tu- mor of skin. It usually occcurs in elderly patients and in neck-head region. It is a rarely seen cancer and goes with high local and regional recurrence and frequent distant me- tastases. In the present report, a case of Merkel cell carci- noma who had undergone postoperative radiotherapy due to remnant tumor after surgical excision but had regional recurrence and had distant metastasis eight months later is presented. The role of radiotherapy and chemotherapy in the treatment of Merkel cell carcinoma, which recurs fre- quently locally and regionally and has distant metastasis, as in our case, is discussed in view of the information in literature.
