Özet
Prostatik intraepitelyal neoplazi (PIN), prostatik duktus ve asinuslarda stromal invazyon olmaksızın morfolojik kanser özellikleri ile karakterize bir lezyondur. Yüksek grade PIN prostat kanseri prekürsörü olarak kabul edilmektedir. Bu çalışmada benign prostat hiperplazisi için standart operasyon geçiren olgularda PIN insidansı ve bu lezyonun yaş, serum prostat spesifik antijen (PSA) düzeyi, prostat spesifik antijen dansitesi (PSAD) ve prostat ağırlığıyla ilişkisi incelenmiştir. Toplam 66 olgunun, 14'ü düşük ve 6'sı yüksekgracte olmak üzere 20'sinde PIN saptanmıştır. Benign prostat hiperplazisi ile birlikte PIN bulunan ve bulunmayan gruplar arasında yaş, prostat ağırlığı, PSA ve PSAD bakımından anlamlı fark saptanmamıştır (p>0.05). Yüksek ve düşük grade PIN grupları arasında PSA düzeyleri anlamlı olarak farklıdır (p= 0.039), ancak PSAD gözönüne alındığında bu gruplar arasında anlamlı fark yoktur (p= 0.381). Benign prostat hiperplazili olgularda PIN insidansı oldukça yüksektir. Özellikle yüksek grade PIN varlığında PSA ve biyopsi ile izlem önemlidir.
Abstract
The Correlation of Prostatic Intraepithelial Neoplasia (PIN) Associated With Benign Prostatic Hyperplasia With Age, Prostate Specific Antigen, Prostat Specific Antigen Density and Prostate Volume- Prostatic intraepithelial neoplasia (PIN) is characterized by the morphologic cancer signs in prostatic ducts and acini without stromal invasion. High grade PIN is accepted as a precursor of prostate cancer. In this study, we evaluated the incidence of PIN with relevance to the age, serum prostate specific antigen (PSA) level, prostate specific antigen density (PSAD) and prostate weight of the patients who underwent prostatectomy for benign disease. Of 66 patients 20 had PIN in their prostatectomy specimens, 14 with low grade and 6 with high grade lesions. There were no differences between the patient groups with and without accompanying PIN with regard to the age, prostatic weight, PSA levels and PSAD (p>0.05). While the PSA levels were found to be statistically different between high grade and low grade PIN groups (p= 0.039), the difference disappeared when PSAD was taken into consideration (p= 0.381). The PIN incidence is high in benign prostatic hyperplasia patients. A meticulous followup with PSA and prostatic biopsies seems crucial in patients with high grade lesions
