Özet
Agn nedeniyle yapilan varikoselektomi sonuclannin gozden gecirilmesi. Hastalar ve Yontem: Mayis 1998 ile Ocak 1999 arasinda klinigimizde varikosel icin ameliyat edilen 104 hastanin operasyon nedenleri belirlendi. Agn nedeniyleyarikoselektomi yapilan 34 hasta prospektif olarak operasyon sonuglan a?isindan degerlendirildi. Bu hastaiarm preoperatif donemde ve postoperatif ikinci ayin sonunda agn siddetleri viziiel analog skala (VAS) ile saptandi. Agn yakinmalannin ozellikleri, varikosel dereceleri ve varikoselin lokalizasyonlan acisindan degerlendirilen olgulann.tedavi sonuclan postoperatif ikinci ayin sonunda VAS degerindeki azalmaya gore 90k iyi, iyi, orta ve kotu olarak ayrilduBu/gu/a/v Agn nedeniyle opere edilen hastalann arasinda graded (41 olgu) ve grade2 (14 olgu) varikcjJBelli olgulann sayisi grade 1 olgu sayisindan (5 olgu) istatistiksel olarak daha fazla idi (sirasiyla, p< 0.001, p< 0.05). Ayni sekilde, ipsilateral testis atrofisi nedeniyle opere edilen hastalardan grade 2 (7 olgu) ve grade 3 (14 olgu) varikoseli olanlann sayisi,grade 1 (1 olgu) varikoseli olan olgu sayisindan daha fazla idi (sirasiyla, p< 0.05, p< 0.001). Ortalama preoperatif agn skoru 5.62±1.18, postoperatif agn skoru 1.32±2.22 olarak belirlendi (p< 0.01). Tedaviye 50k iyi ve iyi yanit veren olgular (toplam 27 olgu) ile orta ve kotu yanit verenlerden (toplam 7 olgu) anlamli olarak yuksek idi (p< 0.001). Sonug: Varikosele bagli agn yakinmasi olan hastalarda varikoselektominin basarili birtedavi modalitesi oldugu dusunulmustur.
Abstract
Outcome Of Varicocelectomies Done Because Of Pain- Objective: To review the outcome of varicocelectomies done because of pain. Patients and Methods: The causes of varicocelectomies in 104 patients opeated between May 1998 and January 1999 were determined. We also prospectively evaluated surgical outcome of'34 patients who underwent varicocelectomies because of pain. The symptom severity of patients operated for pain was determined by using the visual analog scale (VAS) preoperatively and at the end of the second postoperatif month. Cases evaluated with the pain features, varicocele grades and localizations were grouped in to excellent; good; and moderate or poor outcome groups according to the VAS value decrease postoperatively. Results: Among the patients operated for pain, the number of cases who had grade 3 (41 cases) and grade 2 (14 cases) varkoceles was higher than the number of those who had grade 1 (5 cases) varicocele (p< 0.001, p< 0.05). Similarly, the number of patients operated for ipsilateral testicular atrophy with grade 2 (7 cases) and grade 3 (14 cases) varkoceles was significantly higher than the number of those with grade 1 (1 case) varicocele (p< 0.05, p< 0.001). The mean preopeative pain score was 5.62±1.18, and the mean postoperative score xvas 1.32±2.22 (p< 0.01). The number of patients in the excellent or good response groups (total, 27 cases) was significantly higher than the number of patients in the moderate or poor response (total, 7 cases) groups (p< 0.001). Conclusion: Surgical treatment attains successful results in varicocele patients presenting predominantly with pain.
